%0 Journal Article %T 金黄色葡萄球菌血行感染对外周血血小板数量影响的研究 %T Effects of bloodstream infection of Staphylococcus aureus on platelet counts in peripheral blood %A 陈雅文,顾军娟,李一荣 %A CHEN,Ya wen %A GU,Jun juan %A LI,Yi rong %J 中国临床新医学 %J CHINESE JOURNAL OF NEW CLINICAL MEDICINE %@ 1674-3806 %V 12 %N 11 %D 2019 %P 1176-1180 %K 金黄色葡萄球菌;血小板;感染;聚集 %K Staphylococcus aureus;Platelet;Infection;Aggregation %X [摘要] 目的 研究金黄色葡萄球菌(简称金葡菌)血行感染对外周血血小板(PLT)数量的影响。方法 选择该院2014-01~2016-12收治的328例金葡菌感染患者作为研究对象,其中血行感染(SAB)81例,呼吸道感染(SAR)103例,伤口局部感染(SAW)94例以及泌尿道感染(SAU)50例。分析不同感染情况下PLT与血小板平均体积(MPV)的变化情况,以及SAB组感染控制后PLT变化情况与其他临床及实验室参数的关联性。结果 感染控制后,SAB组、SAR组及SAW组的PLT水平均高于感染控制前,MPV水平均低于感染控制前,差异有统计学意义(P<0.05);SAU组的PLT和MPV水平在金葡菌感染控制前后比较差异无统计学意义(P>0.05)。感染控制后,SAB组PLT变化情况与感染控制所需时间和MPV变化率存在显著关联性(P<0.05)。相关性分析结果显示,感染控制后PLT变化率与MPV变化率呈负相关(P<0.05),感染控制后PLT变化率与感染控制所需时间呈正相关(P<0.05)。结论 金葡菌血行感染会导致患者外周血PLT减少,感染控制后PLT变化率与MPV变化率呈负相关,这种变化与PLT的活化和聚集具有关联。 %X [Abstract] Objective To investigate the effects of bloodstream infection of Staphylococcus aureus on platelet(PLT) counts in peripheral blood. Methods Three hundred and twenty-eight cases of Staphylococcus aureus infection in our hospital from January 2014 to December 2016 were selected as the study subjects, including 81 cases of bloodstream infection(SAB), 103 cases of respiratory infection(SAR), 94 cases of wound local infection(SAW) and 50 cases of urinary tract infection(SAU). The changes of PLT and mean platelet volume(MPV) under different infection conditions were analyzed, and the correlations of other clinical and laboratory parameters with different PLT changes after infection control in the SAB group were investigated. Results After infection control, the levels of PLT in the SAB group, the SAR group and the SAW group were significantly higher and the levels of MPV were significantly lower than that those before infection control(P<0.05). There were no significant differences in the levels of PLT and MPV before and after control of Staphylococcus aureus infection in the SAU group(P>0.05). After infection control, the PLT changes in the SAB group were significantly correlated with the time needed for infection control and the change rate of MPV(P<0.05). The results of correlation analysis showed that there was a negative correlation between PLT change rate and MPV change rate after infection control(P<0.05), and a positive correlation between PLT change rate and infection control time(P<0.05). Conclusion Bloodstream infection of Staphylococcus aureus can lead to a decrease in the number of peripheral blood PLT in patients, and the change rate of PLT after infection control is negatively correlated with MPV change rate, which is related to the activation and aggregation of PLT. %R 10.3969/j.issn.1674-3806.2019.11.07 %U http://www.zglcxyxzz.com/ch/reader/view_abstract.aspx %1 JIS Version 3.0.0
