| ［摘要］ 目的 探讨四妙散加减辅助关节镜下清理术治疗慢性膝关节滑膜炎的疗效及安全性。方法 收集2014-06～2016-05住院治疗的慢性膝关节炎患者101例。采用抽签法将患者分为观察组51例和对照组50例。对照组采用关节镜下清理术治疗。观察组在关节镜下清理术后给予四妙散加减方治疗。比较两组患者术前、术后1个月时的疼痛程度、关节肿胀评分情况，术前、术后3个月时的膝关节功能变化。记录治疗期间患者的不良反应情况及对比两组术后3个月时的临床疗效。结果 观察组的临床疗效较对照组明显提高(P<0.05)；术后1个月，观察组的疼痛程度视觉模拟评分（VAS）较对照组明显降低，关节肿胀评分较对照组明显提高(P<0.05)；术后3个月，观察组的膝关节功能（Lysholm）评分较对照组明显提高(P<0.05)。对照组无不良反应发生，观察组治疗期间有7例出现胃部不适，对症处理后好转；两组不良反应差异有统计学意义(P<0.05)。结论 关节镜下清理术后辅以四妙散加减方治疗慢性膝关节滑膜炎，能有效减轻术后关节疼痛、肿胀等症状，促进膝关节功能恢复，提高临床疗效，且安全可靠，值得临床推广应用。 |

| ［Abstract］ Objective To investigate the effect of modified Simiao powder assisted arthroscopic debridement on treating chronic gonarthromeningitis.Methods 101 patients with chronic knee osteoarthritis who were treated in our hospital from June 2014 to May 2016 were collected. They were divided into the observation group(n=51) and the control group(n=50) by drawing lots. The control group was treated with arthroscopic debridement. The observation group was treated with modified Simiao powder after arthroscopy debridement. The degree of pain, the scores of joint swelling before surgery and one month after surgery, and the knee function before surgery and three months after surgery were compared between the two groups. The adverse reactions during treatment were recorded and the clinical efficacy at postoperative three months were compared between the two groups.Results Compared with that in the control group, the clinical curative effect in the observation group increased significantly(P<0.05); Compared with those in the control group, at postoperative one month, the VAS scores in the observation group reduced significantly, and the scores of joint swelling improved significantly(P<0.05); Compared with those in the control group at postoperative three months, the Lysholm scores in the observation group increased significantly(P<0.05). There was no adverse reaction in the control group. There were 7 patients with stomach discomfort which was improved after symptomatic treatment in the observation group during the treatment period. There were significant differences in the adverse drug reactions between the two groups.Conclusion The modified Simiao powder assisted arthroscopic debridement can effectively reduce the postoperative pain, swelling and other symptoms, and promote the recovery of knee joint function, and improve the clinical curative effect in the treatment of chronic gonarthromeningitis. |
