| ［摘要］ 目的 对比彩色多普勒超声经腹部检查与经阴道检查在瘢痕妊娠（scar pregnancy,CSP）诊断中应用价值，旨在为今后临床CSP诊断提供参考与借鉴。方法 选取2015-12～2017-11该院收治疑似CSP患者58例，所有患者均在医师指导下分别行彩色多普勒超声经腹部检查与阴道彩色多普勒超声检查，其检查结果与手术后病理结果比较，对比两种彩色多普勒超声检查诊断灵敏度、特异度及符合率。结果 经阴道超声在瘢痕子宫中诊断灵敏度、特异度均高于经腹部超声超声诊断（97.37% vs 81.58%，95.00% vs 75.00%，P<0.05）。38例患者经腹部彩色多普勒超声检查，其囊样回声、孕囊、混杂回声检出总例数31例，而经阴道彩色多普勒超声检查其检出总例数37例，经阴道彩色多普勒超声检查诊断符合率显著高于经腹部彩色多普勒超声检查，差异有统计学意义（P<0.05）。结论 经阴道彩色多普勒超声检查在CSP诊断中具有较高的诊断符合率，比经腹部彩色多普勒超声检查更具临床优越性，对CSP早期诊疗有十分重要的意义。 |

| ［Abstract］ Objective To compare the diagnostic value of Doppler ultrasound between abdominal examination and vaginal examination in scar pregnancy(CSP). Methods 58 cases of suspected CSP patients in our hospital during December 2015 and November 2017 were selected as the study subjects. All the patients received color Doppler ultrasound and abdominal and vaginal color Doppler ultrasound examination under the guidance of the physicians. The ultrasonic results were compared with the postoperative pathological results. The diagnostic sensitivity, specificity and coincidence rates were compared between the two examination methods of color Doppler ultrasound. Results The diagnostic sensitivity and specificity rates of transvaginal sonography in diagnosis of uterine scar were higher than those of transabdominal ultrasound ultrasound（97.37% vs 81.58%, 95.00% vs 75.00%, P<0.05）. Of the 38 patients examined by abdominal color Doppler ultrasound, cystic echo, gestational sac, mixed echoes were detected in 31 cases, and the results were detected in 37 cases by transvaginal color Doppler ultrasound. The diagnostic coincidence rate of transvaginal color Doppler ultrasound examination was significantly higher than that of transabdominal color Doppler ultrasound examination(P<0.05). Conclusion Transvaginal color Doppler ultrasonography has a higher diagnostic accuracy rate than transabdominal color Doppler ultrasound in diagnosis of CSP. |
