Pro-BNP与儿童急性肺损伤和急性呼吸窘迫综合征的相关性研究

摘要:
[摘要] 目的 探讨脑钠钛前体(pro-BNP)与儿童急性肺损伤(ALI)和急性呼吸窘迫综合征(ARDS)相关指标的变化关系。方法 选取该院2015-03~2016-07入住的30例ALI与ARDS患儿作为观察组,并以患儿血浆pro-BNP水平分为A区(低水平),B区(中水平),C区(高水平);同时选取同期30例心源性疾病伴呼吸困难患儿作为对照组。检测并对比两组患儿pro-BNP水平;分别对两组患儿肺损伤程度(LIS)与疾病危重程度(PCIS)评分并对比分析其与患儿pro-BNP水平的关系及患儿死亡影响因素。结果 观察组患儿血清pro-BNP水平明显高于对照组(P<0.05),而对照组心源性疾病患儿发生肺水肿者其pro-BNP则高于观察组ALI与ARDS患儿水平(P<0.05);观察组中死亡患儿、PCIS≤70分患儿与LIS>2.5分的患儿pro-BNP水平均较高;ALI与ARDS患儿LIS、1个月生存情况与血清pro-BNP水平呈正相关,患儿PCIS与其呈负相关;pro-BNP水平B与C区间是ALI/ARDS患儿死亡的影响因素。结论 在ALI与ARDS患儿的早期诊断与预后评估中采用pro-BNP检测,可评估患儿病情的严重程度与预后,为其早期确诊与治疗提供参照,有较高的应用价值。

Research on correlation between pro-BNP and acute lung injury and acute respiratory distress syndrome in children

Abstract:
[Abstract] Objective To investigate the relationship between pro-brain natriuretic peptide(pro-BNP) and acute lung injury(ALI) and acute respiratory distress syndrome(ARDS) in children. Methods 30 children with ALI and ARDS who were admitted to our hospital from March 2015 to July 2016 were selected as the observation group. The observation group was divided into three subgroups: A(low level subgroup), B(medium level subgroup) and C(high level subgroup) by the different plasma pro-BNP levels, At the same time, 30 patients with heart disease complicated with dyspnea were selected as the control group. The levels of pro-BNP and the scores of lung injury(LIS) and disease severity(PCIS) were compared between the observation group and the control group. The correlation between pro-BNP and acute lung injury, acute respiratory distress syndrome and the factors of death were analyzed. Results The serum level of pro-BNP in the observation group was significantly higher than that in the control group(P<0.05), while the level of pro-BNP in the cardiac disease children with pulmonary edema in the control group were higher than that in the observation group(P<0.05). The level of pro-BNP in the children of the observation group who were dead, and whose PCIS scores ≤70 and LIS scores >2.5 was high. The LIS scores of the observation group and one-month survival rate were positively correlated with the level of serum pro-BNP, while PCIS scores were negatively correlated with the level of serum pro-BNP. The pro-BNP levels in B and C intervals were the factors of death in the children with ALI/ARDS. Conclusion Pro-BNP can evaluate the severity of illness and the prognosis, and provide reference for early diagnosis and treatment in the patients with ALI and ARDS.
