摘要:
[摘要] 目的 分析抗-cE合并抗-Mur致血型鉴定及交叉配血困难的特征。方法 对2例受检者血标本进行血型鉴定。其血清与已知血型的试剂红细胞,在盐水、木瓜酶、凝聚胺、抗球蛋白介质中反应,鉴定抗体的特异性。采用盐水法、凝聚胺法、微柱凝胶法进行交叉配血试验。结果 病例1血清中存在IgM与IgG混合型抗-cE、IgM型抗-Mur,导致血型鉴定及交叉配血困难。病例2血清中存在IgG型抗-cE、IgM型抗-Mur,导致血型鉴定及交叉配血困难。结论 抗-cE合并抗-Mur可致血型鉴定及配血困难,对该类患者进行不规则抗体检测和交叉配血过程中必须采用多种介质,并确认供血者对应抗原为阴性,防止假阴性漏检导致溶血性输血反应的发生。

Abstract:
[Abstract] Objective To analyze the characteristics of anti-cE combined with anti-Mur causing difficulty in tests of blood typing and cross matching. Methods Blood typing test was performed in two patients. The serum of the two patients reacted with reagent red blood cells whose blood groups were known in the phase of saline, papain, polyamine and antiglobulin, then the specificity of the antibodies was identified. Cross matching tests were performed by methods of saline phase, polyamine phase and micro gel column technique. Results We found that anti-cE of IgM and IgG type and anti-Mur of IgM type in serum of case 1, anti-cE of IgG type and anti-Mur of IgM type in serum of case 2, caused difficulty in tests of blood group identification and cross matching. Conclusion Anti-cE combined with anti-Mur may lead to difficulty in blood typing and matching for the patients with these two antibodies, and various methods should be used in the process of irregular antibody detection and cross matching, and the corresponding antigens of the blood provided to the patients should be confirmed as negative, to prevent hemolytic transfusion reaction caused by the results of false negative.
