腓总神经松解联合伸直优先法在严重外翻膝关节置换术中的应用效果观察

摘要:
[摘要] 目的 观察腓总神经松解联合伸直优先法在严重外翻膝关节置换术中的临床应用效果。方法 选择在该院接受膝关节置换术治疗的严重膝外翻患者34例,患者均采取腓总神经松解联合伸直优先截骨法治疗,分析患者手术情况,比较患者术前、术后2周、6周关节活动度变化以及术前、术后6个月、术后12个月膝关节功能情况。结果 34例术后均无神经损伤、感染以及肺栓塞等严重并发症。术后2周、术后6周患者关节活动度较术前明显增加(P<0.05),且术后6周患者关节活动度明显高于术后2周(P<0.05)。术后6个月、术后12个月患者膝关节评分法(Knee Society Score,KSS)临床评分、KSS功能评分以及视觉模拟评分法(Visual Analogue Scale,VAS)评分均显著高于术前(P<0.05),术后6个月与术后12个月比较差异均无统计学意义(P>0.05)。2例患者出现下肢肌间静脉以及深静脉血栓。结论 腓总神经松解联合伸直优先法在严重外翻膝关节置换术中应用可有效改善患者术后关节活动度以及膝关节功能,缓解患者膝关节疼痛,手术安全,有较好的临床应用价值。

Abstract:
[Abstract] Objective To observe the clinical effect of peroneal neurolysis combined with straightening priority on severe valgus knee arthroplasty. Methods Thirty-four patients receiving severe valgus knee arthroplasty were collected in our hospital. The patients were treated with peroneal nerve neurolysis plus extension osteotomy and straightening priority treatment. The changes of joint activity were compared before and 2 weeks, 6 weeks after operation. The changes of knee joint function were compared before and 6 months and 12 months after operation. Results No serious complications such as nerve injury, infection and pulmonary embolism were found after operation. The activity of joints improved significantly 2 weeks and 6 weeks after operation(P<0.05), and the activity of joints 6 weeks after operation was significantly higher than that 2 weeks after operation(P<0.05). 6 months and 12 months after operation, the Knee Society Score(KSS) clinical scores, KSS function scores and Visual Analogue Scale(VAS) scores were significantly higher than those before operation(P<0.05). There were no significant differences in the above scores between 6 and 12 months after operation(P>0.05). Two cases occurred lower extremity intermuscular vein and deep vein thrombosis. Conclusion Peroneal nerve neurolysis plus straightening priority treatment has good clinical application value for severe valgus knee arthroplasty. It is safe can effectively improve the postoperative joint mobility and the knee joint function, and relieve the patients′ knee joint pain.
