血清NGAL水平变化在大鼠不同程度急性肾损伤中的早期诊断价值

[摘要] 目的 监测不同程度急性肾损伤(AKI)大鼠血清中性粒细胞明胶酶相关脂质运载蛋白(NGAL)水平变化,探讨NGAL在AKI中的早期诊断价值。方法 将40只雄性Wistar大鼠分为假手术组、轻度损伤组、中度损伤组和重度损伤组,每组10只,分别制备不同程度肾损伤模型,在0 h、2 h、6 h、12 h、24 h时间点进行血清NGAL测定,分析NGAL水平变化在不同时间点的诊断价值。结果 (1)不同肾损伤程度组NGAL水平比较,差异无统计学意义(P>0.05);假手术组、轻度损伤组NGAL水平在不同时间点上差异无统计学意义(P>0.05);中度损伤组、重度损伤组NGAL水平在不同时间点上差异有统计学意义(P<0.05);组别和时间点之间无交互作用(P>0.05);(2)ROC曲线下面积(AUC)2 h、6 h、12 h、24 h分别为0.855、0.413、0.210、0.370。结论 大鼠中度及重度AKI的NGAL水平变化趋势相同,NGAL表达水平在损伤后2 h出现高峰,可以将其作为AKI早期诊断的最佳时间点;ROC曲线进一步显示了2 h时间点在大鼠危重AKI早期诊断中具有很高的价值。

The early diagnostic value of NGAL in different degree of acute kidney injury in rats

Abstract:
[Abstract] Objective To monitor the changes of neutrophil gelatinase-associated lipocalin(NGAL) in the serum of rats with acute kidney injury(AKI) and to explore the value of NGAL in the early diagnosis of AKI. Methods Forty male Wistar rats were divided into sham-operation group, mild injury group, moderate injury group and severe injury group, with 10 cases in each group. The models of renal injury were made at different time points. Serum NGAL was detected at 0 h, 2 h, 6 h, 12 h and 24 h, and the diagnostic value of NGAL at different time points was analyzed. Results (1)There were no significant differences in the NGAL levels among the groups with different degrees of renal injury(P>0.05). There were no significant differences in the NGAL levels between the sham-operation group and mild injury group at different time points(P>0.05), while the NGAL levels of the moderate injury group and the severe injury group were statistically different at different time points(P<0.05). There was no interaction between groups and time points(P>0.05). (2)AUCs were 0.855, 0.413, 0.210 and 0.370 at 2 h, 6 h, 12 h and 24 h respectively. Conclusion The changes of NGAL show the same trend in the rats with moderate and severe AKI. NGAL expression peaks at 2 h after injury, which can be used as the best time point for early diagnosis of AKI. The ROC curve further shows that the time point of 2 h associated has great value in the early diagnosis of critical AKI in rats.
