布地奈德对AECOPD患者肺功能 动脉血气指标 炎性因子及临床症状的影响观察

摘要:
[摘要] 目的 观察布地奈德雾化吸入治疗对慢性阻塞性肺疾病急性加重期(AECOPD)患者肺功能、动脉血气指标、炎性因子及临床症状的影响。方法 选取2016-01~2016-08该院100例AECOPD患者,按随机数字表法分为观察组和对照组各50例。两组均给予抗感染、祛痰、吸氧及支气管扩张剂等基础治疗,观察组同时给予布地奈德进行雾化吸入治疗,疗程7 d。对比两组肺功能、动脉血气指标变化、炎性因子及临床症状缓解情况。结果 治疗后,观察组第一秒用力呼气容积占预计值(FEV1%pred)、第一秒用力呼气容积/肺活量(FEV1/FVC)、动脉血氧分压(PaO2)、pH测定值均高于对照组(P<0.05),血清白细胞介素-8(IL-8)、白细胞介素-6(IL-6)、白细胞介素-32(IL-32)、肿瘤坏死因子-α(TNF-α)测定值均低于对照组(P<0.05),咳嗽、咳痰及肺部听诊评分均低于对照组(P<0.05)。结论 布地奈德雾化吸入治疗AECOPD能进一步缓解患者的临床症状,改善肺功能及血气水平。

Abstract:
[Abstract] Objective To observe the effects of budesonide aerosol inhalation on lung fanction, blood gas index, inflammatory factors and clinical symptoms in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD). Methods From January 2016 to August 2016, 100 AECOPD patients in our hospital were randomly divided into observation group and control group, with 50 cases in each group by randon number table method. Both of the two groups were given anti-infection, oxygen, expectorant and bronchodilator therapy, and the observation group was given additional budesonide aerosol inhalation therapy for 7 days. The changes of lung function, blood gas index, inflammatory factors and clinical symptomatic remissions were compared between the two groups. Results After treatment, the values of FEV1%pred, FEV1/FVC, PaO2 and pH in the observation group were significantly higher than those in the control group(P<0.05) and the serum levels of IL-8, IL-6, IL-32 and TNF-α in the observation group were significantly lower than those in the control group(P<0.05); the scores of cough, expectoration and lung auscultation in the observation group were significantly lower than those in the control group(P<0.05). Conclusion Budesonide aerosol inhalation can further alleviate the clinical symptoms of the patients with AECOPD,and improve their lung function and blood gas.
