钙离子结合蛋白S100A4在胰头癌组织中的表达及其对患者生存预后的影响分析

[摘要] 目的 探讨钙离子结合蛋白S100A4在胰头癌组织中的表达及其对患者生存预后的影响。方法 选择2000-01-01~2007-12-01该院手术切除的胰头癌组织38例,另取11例癌旁正常胰腺组织作为对照。采用免疫组化检测组织样本中S100A4蛋白的表达水平,分析其与胰头癌临床病理特征及其对患者生存预后的影响。结果 胰头癌组织S100A4蛋白阳性表达率为71.05%(27/38),癌旁正常胰腺组织未见S100A4蛋白阳性表达,两组阳性率比较差异有统计学意义(χ2=14.654,P=0.000)。胰头癌组织S100A4蛋白阳性组与阴性组在肿瘤直径、是否远处转移、TNM分期及有无淋巴结转移方面比较差异有统计学意义(P<0.05)。胰头癌患者S100A4阴性组的生存预后显著优于阳性组(P<0.05)。Cox回归分析结果显示,以TNM Ⅳ期为参照,TNM Ⅰ期和TNM Ⅱ期是患者生存预后的保护因素(P<0.05),而S100A4阳性表达是胰头癌患者预后的危险因素(P<0.05)。结论 胰头癌组织S100A4蛋白表达升高与胰头癌的发生、发展及预后密切相关。

[Abstract] Objective To investigate the expression of calcium-binding protein S100A4 in the tissues of pancreatic head carcinoma and its impact on survival and prognosis of pancreatic head carcinoma patients. Methods Thirty-eight tissue samples of pancreatic head carcinoma resected in our hospital From January 1, 2000 to December 1, 2007 were selected, and 11 normal pancreatic tissues adjacent to the cancer were taken as controls. The expression level of S100A4 protein in the tissue samples was detected by immunohistochemistry. The relationship between the expression level of S100A4 protein and the clinicopathological features of pancreatic head carcinoma and its influence on survival and prognosis of the patients were analyzed. Results The positive expression rate of S100A4 protein in the pancreatic head carcinoma tissues was 71.05%(27/38), while no positive expression of S100A4 protein was found in the normal pancreatic tissues adjacent to the cancer, and the positive rate was significantly different between the two groups(χ2 =14.654, P=0.000). There were significant differences between the S100A4 protein-positive group and the S100A4 protein- negative group of pancreatic head carcinoma tissues in tumor diameter, whether distant metastasis, TNM stage and with or without lymph node metastasis(P<0.05). The survival prognosis of the patients with pancreatic head carcinoma in the S100A4 protein-negative group was significantly better than that in the S100A4 protein-positive group(P<0.05). The results of Cox regression analysis showed that TNM stageⅠand TNM stage Ⅱ were the protective factors of survival prognosis for the patients when TNM stage Ⅳ was taken as reference(P<0.05), and S100A4 positive expression was a risk factor of prognosis for the patients with pancreatic head carcinoma(P<0.05). Conclusion The elevated expression of S100A4 protein in pancreatic head carcinoma tissues is closely related to the occurrence, development and prognosis of pancreatic head carcinoma.
