伞式单孔免气腹技术行腹腔镜胆囊切除术的动物实验研究

[摘要] 目的 探讨自行研发的伞式单孔免气腹装置在腹腔镜胆囊切除术中的应用效果。方法 将39只实验猪随机分为实验组(n=20,应用伞式单孔免气腹装置进行腹腔镜胆囊切除术)和对照组(n=19,应用常规气腹进行腹腔镜胆囊切除术)。比较两组的手术时间、术中出血量、术后超敏C反应蛋白(hs-CRP)、手术切口长度及术中并发症发生情况。结果 两组手术均顺利完成,均无中转开腹手术,无术中大出血、器官损伤等严重并发症发生。实验组手术时间较对照组长,差异有统计学意义(P<0.05)。两组在术中出血量、术前后hs-CRP水平和手术切口长度比较差异无统计学意义(P>0.05)。结论 与常规气腹手术比较,应用伞式单孔免气腹装置行腹腔镜胆囊切除手术安全可行,且手术伤口位于脐部,美容效果更好。

An animal experimental study on laparoscopic cholecystectomy using umbrella-type single-port gasless laparoscopy

Abstract:
[Abstract] Objective To explore the application effect of self-developed umbrella-type gasless device on laparoscopic cholecystectomy with single incision. Methods Thirty-nine experimental pigs were randomly divided into the experimental group(n=20, laparoscopic cholecystectomy using umbrella-type gasless device with single incision) and the control group(n=19, laparoscopic cholecystectomy using conventional pneumoperitoneum). The operation time, intraoperative blood loss, postoperative hypersensitive C-reactive protein(hs-CRP), length of surgical incision and intraoperative complications were compared between the two groups. Results The operations in both groups were completed successfully, and there were no conversion to open surgery, and no serious complications such as major bleeding and organ damage during the operations. The operation time of the experimental group was longer than that of the control group, and the difference was statistically significant(P<0.05). There were no significant differences between the two groups in the intraoperative blood loss, the levels of hs-CRP before and after operation, and the length of surgical incision(P>0.05). Conclusion Compared with the conventional pneumoperitoneum surgery, the self-developed umbrella-type gasless device is safe and feasible for laparoscopic cholecystectomy with single incision, and the surgical wound is located in the umbilicus when the self-developed umbrella-type gasless device with single incision is used, and its cosmetic effect is better.
