经直肠应变力弹性成像评估良性前列腺增生组织硬度变化及膀胱出口梗阻程度的应用价值

摘要:
[摘要] 目的 探讨经直肠应变力弹性成像(TRTE)评估良性前列腺增生(BPH)组织硬度变化及膀胱出口梗阻程度的应用价值,分析前列腺组织硬度指标与膀胱出口梗阻程度的相关性。方法 选择2018-06~2020-02在该院男科就诊并行TRTE检查的患者111例作为研究对象,其中BPH患者81例(BPH组),非BPH患者30例(非BPH组)。应用TRTE获取患者前列腺弹性图像,比较两组弹性评分、外腺应变、内腺应变和应变比(SR)。采用Spearman秩相关分析BPH组外腺应变、内腺应变、SR及弹性评分与AG值的相关性。结果 BPH组外腺应变、SR及弹性评分显著高于非BPH组(P<0.05)。BPH组内腺应变[0.13(0.10,0.17)]小于外腺应变[0.32(0.24,0.42)],差异有统计学意义(Z=-7.569,P=0.000)。Spearman秩相关分析结果显示,BPH组AG值与内腺应变呈负相关(P<0.05),与SR和弹性评分呈正相关(P<0.05)。结论 TRTE可用于评估前列腺组织的弹性质地,BPH的发生会使前列腺组织硬度增加,内腺应变、SR、弹性评分与膀胱出口梗阻程度具有相关性。

Application value of transrectal real-time tissue elastography in evaluation of the change of tissue hardness and the degree of bladder outlet obstruction in benign prostatic hyperplasia

Abstract:
[Abstract] Objective To explore the application value of transrectal real-time tissue elastography(TRTE) in evaluation of the change of tissue hardness and the degree of bladder outlet obstruction in benign prostatic hyperplasia(BPH), and to analyze the correlation between the indexes of prostate tissue hardness and the degree of bladder outlet obstruction. Methods A total of 111 patients visiting the Department of Andrology, the Fifth Affiliated Hospital of Guangxi Medical University and undergoing TRTE examination from June 2018 to February 2020 were selected as the study subjects, including 81 patients with BPH(BPH group) and 30 patients without BPH(non-BPH group). The patients′ prostate elasticity images were obtained by TRTE, and the elasticity scores, strain of the external glands, strain of the internal glands and strain ratio(SR) were compared between the two groups. The correlation between AG value and the strain of the external glands, strain of the internal glands, SR and elasticity scores was analyzed by Spearman rank correlation analysis in the BPH group. Results The strain of the external glands, SR and elasticity scores in the BPH group were significantly higher than those in the non-BPH group(P<0.05). In the BPH group, the strain of the internal glands [0.13(0.10, 0.17)] was smaller than that of the external glands [0.32(0.24, 0.42)], and the difference was statistically significant(Z=-7.569, P=0.000). The results of Spearman rank correlation analysis showed that the AG value was negatively correlated with the strain of the internal glands(P<0.05), and positively correlated with the SR and elasticity scores(P<0.05) in the BPH group. Conclusion TRTE can be used to evaluate the elastic texture of prostate tissues. The occurrence of BPH increases the hardness of prostate tissues. The strain of the internal glands, SR and elasticity score are correlated with the degree of bladder outlet obstruction.
