人工髋关节置换术后无菌性松动患者血清和假体界膜组织中IL-34 RANKL和OPG的表达水平及其临床意义

[摘要] 目的 研究人工髋关节置换术后无菌性松动患者血清和假体界膜组织中白细胞介素-34(IL-34)、核因子κB受体活化因子配体(RANKL)和骨保护素(OPG)的表达水平及其临床意义。方法 选择2018-10~2020-06该院因髋关节置换术后髋关节假体发生无菌性松动而需进行翻修的患者10例为研究组,另纳入同期于该院因股骨头坏死或股骨颈骨折行初次全髋关节置换术的患者10例为对照组。术前,两组均取静脉血5 ml;术中,研究组取髋臼杯周围的界膜组织,对照组取髋臼周围筋膜和关节囊组织,备用检测。比较两组组织样本和血清中IL-34、RANKL和OPG的表达情况。结果 酶联免疫吸附试验(ELISA)检测结果显示,研究组血清IL-34、RANKL及OPG水平显著高于对照组(P<0.05)。实时定量聚合酶链式反应(RT-PCR)检测结果显示,研究组组织样本IL-34、RANKL及OPG的mRNA的相对表达水平显著高于对照组,且研究组RANKL/OPG值为(1.21±0.36),对照组为(0.59±0.24),差异有统计学意义(P<0.05)。结论 人工髋关节置换术后无菌性松动患者血清和假体界膜组织中IL-34表现为高表达,其可能与RANKL、OPG共同参与了无菌性松动的发病过程。

Expressions and clinical significances of IL-34, RANKL and OPG in the serum and prosthetic boundary membrane tissues of the patients with aseptic loosening after artificial hip replacement

Abstract:
[Abstract] Objective To investigate the expressions and clinical significances of interleukin-34(IL-34), receptor activator of nuclear factor κB ligand(RANKL), and osteoprotegerin(OPG) in the serum and prosthetic boundary membrane tissues of the patients with aseptic loosening after artificial hip replacement. Methods From October 2018 to June 2020, 10 patients in our hospital who needed revision of hip prosthesis due to aseptic loosening of hip prosthesis after hip replacement surgery were selected as research group, and 10 patients who underwent initial total hip replacement due to femoral head necrosis or femoral neck fracture in our hospital during the same period were included as control group. Before operation, 5 milliliters of venous blood were taken from both groups; during operation, the boundary membrane tissues around the acetabular cup were taken from the research group, and the fascia and joint capsule tissues around the acetabulum were taken from the control group for further examination. The expressions of IL-34, RANKL and OPG in the tissue samples and serum were compared between the two groups. Results The results of enzyme-linked immunosorbent assay(ELISA) showed that the serum IL-34, RANKL and OPG levels in the research group were significantly higher than those in the control group(P<0.05). The real-time quantitative polymerase chain reaction(RT-PCR) test results showed that the relative expression levels of IL-34, RANKL and OPG mRNA in the tissue samples of the research group were significantly higher than those of the control group, and the RANKL/OPG value was (1.21±0.36) in the research group, and (0.59±0.24) in the control group, with a significant difference between the two groups(P<0.05). Conclusion IL-34 is both highly expressed in the serum and prosthetic boundary membrane tissues of the patients with aseptic loosening after artificial hip replacement, which may be involved in the pathogenesis of aseptic loosening together with RANKL and OPG.
