毛细支气管炎患儿血清MMP-9 TIMP-1及IL-17水平的变化及临床意义

摘要:
[摘要] 目的 探讨毛细支气管炎患儿血清基质金属蛋白酶9(MMP-9)、基质金属蛋白酶组织抑制因子1(TIMP-1)和白细胞介素17(IL-17)水平的变化及其临床意义。方法 选取2018-10~2019-03该院收治的毛细支气管炎患儿56例,将其分为普通组(32例,包括轻度患儿)和严重组(24例,包括中度和重度患儿),并以同期30名门诊体检健康儿童作为对照组。采用酶联免疫吸附法(ELISA)检测三组血清MMP-9、TIMP-1及IL-17水平,探讨MMP-9、TIMP-1、IL-17与毛细支气管炎严重程度的相关性,及其对毛细支气管炎的诊断效能。结果 普通组和严重组血清MMP-9、TIMP-1、IL-17水平高于对照组,差异有统计学意义(P<0.05)。严重组血清MMP-9、TIMP-1、IL-17水平高于普通组,差异有统计学意义(P<0.05)。Spearman相关分析结果显示,MMP-9(rs=0.799)、TIMP-1(rs=0.731)、IL-17(rs=0.756)与疾病严重程度均呈正相关(P<0.05)。ROC曲线分析结果显示,血清MMP-9、TIMP-1、IL-17对毛细支气管炎诊断均有参考意义(P<0.05),其中以MMP-9的曲线下面积(AUC)最大,诊断效能最佳。结论 动态观察患儿血清MMP-9、TIMP-1和IL-17水平可评估疾病的严重程度,且对于毛细支气管炎的早发现、早治疗具有积极意义。

Changes of serum MMP-9, TIMP-1 and IL-17 levels in children with bronchiolitis and their clinical significances

Abstract:
[Abstract] Objective To explore the changes of serum matrix metalloproteinase 9(MMP-9), tissue inhibitor of matrix metalloproteinase 1(TIMP-1) and interleukin-17(IL-17) levels in children with bronchiolitis and their clinical significances. Methods Fifty-six children with bronchiolitis admitted to our hospital from October 2018 to March 2019 were selected and divided into common group(32 cases, including children with mild symptom) and severe group(24 cases, including children with moderate and severe symptoms), and 30 healthy children receiving physical examination in the outpatient department in the same period were taken as the control group. The serum levels of MMP-9, TIMP-1 and IL-17 were detected by enzyme-linked immunosorbent assay(ELISA) in the three groups. The correlation between MMP-9, TIMP-1, IL-17 and the severity of bronchiolitis, and their diagnostic efficacy for bronchiolitis were investigated. Results The serum levels of MMP-9, TIMP-1 and IL-17 in the common group and the severe group were higher than those in the control group, and the differences were statistically significant(P<0.05). The serum levels of MMP-9, TIMP-1 and IL-17 in the severe group were higher than those in the common group, and the differences were statistically significant(P<0.05). Spearman correlation analysis showed that MMP-9(rs=0.799), TIMP-1(rs=0.731) and IL-17(rs=0.756) were positively correlated with the severity of the disease(P<0.05). The results of receiver operating characteristic(ROC) curve analysis showed that the serum MMP-9, TIMP-1 and IL-17 had reference significance for the diagnosis of bronchiolitis(P<0.05), among which the area under curve(AUC) of MMP-9 was the largest and the diagnostic efficiency was the best. Conclusion Dynamic observation of serum MMP-9, TIMP-1 and IL-17 levels in children can evaluate the severity of the disease, and it has positive significance for the early detection and early treatment of bronchiolitis.
