扁桃体鳞状细胞癌中CK7 CK19和p16的表达关系研究

[摘要] 目的 分析细胞角蛋白7(CK7)、细胞角蛋白19(CK19)和p16INK4a蛋白(p16)在扁桃体鳞状细胞癌(TSCC)组织及癌周隐窝上皮和表面上皮中的表达关系,探讨其在人乳头状瘤病毒(HPV)感染相关扁桃体癌变机制中的作用。方法 选择2015-07~2018-01广西医科大学第一附属医院耳鼻喉头颈外科收治的17例TSCC患者的病历资料及组织样本。对TSCC组织样本进行连续石蜡切片,采用免疫组化法检测CK7、CK19、p16的表达情况,观察和分析三者的表达强度、定位以及相互关系。结果 CK7、CK19、p16阳性染色均定位于胞质。在TSCC组织中,呈斑片或弥漫染色,在TSCC癌周正常上皮中,呈斑片染色。CK7主要表达于上皮表层,CK19主要表达于上皮下层和基底细胞。p16主要表达于上皮的下1/3至全层,与CK19部位基本重叠。对于HPV相关性(p16+)TSCC,Kappa一致性分析结果显示,CK7与CK19、CK7与p16以及CK19与p16的检测结果均具有一致性(P<0.05);但对于非HPV相关性TSCC,三个标志物检测结果未显示出显著关联(P>0.05)。对于TSCC癌周隐窝上皮和癌周表面上皮组织,Kappa一致性分析结果显示,CK7与CK19、CK7与p16以及CK19与p16的检测结果均具有一致性(P<0.05)。结论 CK7、CK19与p16的表达与HPV相关性(p16+)TSCC的发生密切相关,可能存在一种“CK7→CK19→p16轴”作用机制。

A study on the relationship of CK7, CK19 and p16 expressions in tonsil squamous cell carcinoma

[Abstract] Objective To analyze the relationship of cytokeratin 7(CK7), cytokeratin 19(CK19) and p16INK4a(p16) expressions in tonsil squamous cell carcinoma(TSCC) tissue and crypt epithelium and surface epithelium of the pericarcinoma, and to explore their roles in the mechanism of human papilloma virus(HPV) infection-mediated tonsillar carcinogenesis. Methods The case data and tissue samples of 17 TSCC patients admitted to the Department of Otorhinolaryngology Head and Neck Surgery, the First Affiliated Hospital of Guangxi Medical University from July 2015 to January 2018 were selected. Serial paraffin sections were performed on the TSCC tissue samples, and immunohistochemistry was used to detect the expressions of CK7, CK19, and p16. The expression intensity, location and relationship of the three markers were observed and analyzed. Results The positive staining of CK7, CK19, and p16 was located in the cytoplasm. In the TSCC tissues, their staining was patchy or diffuse, while in the normal epithelium around the TSCC cancer, their staining was patchy. CK7 was mainly expressed in the epithelial surface, while CK19 was mainly expressed in the subepithelial and basal cells. p16 was mainly expressed in the lower third of the epithelium to the whole layer, and basically overlapped with CK19. For HPV-mediated(p16+) TSCC, the results of Kappa consistency analysis showed that the detection results of CK7 and CK19, CK7 and p16, and CK19 and p16 were consistent(P<0.05); however, for non-HPV-mediated TSCC, the detection results of the three markers did not show a significant correlation(P>0.05). For the pericancerous crypt epithelium tissues and pericancerous surface epithelium tissues around the TSCC cancer, the results of Kappa consistency analysis showed that the detection results of CK7 and CK19, CK7 and p16, and CK19 and p16 were all consistent(P<0.05). Conclusion The expressions of CK7, CK19 and p16 are closely related to the occurrence of HPV-mediated(p16+) TSCC, and there may be a mechanism of “CK7→CK19→p16 axis”.
