成人骨性Ⅱ类高角错(牙合)畸形代偿治疗前后上颌前牙牙根吸收的锥形束CT定量分析

摘要:
[摘要] 目的 采用锥形束CT(CBCT)三维重建技术配合Mimics 20.0软件定量分析成人骨性Ⅱ类高角错(牙合)畸形代偿治疗前后上颌中切牙、侧切牙和尖牙牙根吸收的情况。方法 选择2016年11月至2018年11月于贵阳市口腔医院正畸科实施正畸代偿治疗的成人骨性Ⅱ类高角错(牙合)患者30例。使用Mimics20.0软件对CBCT数据进行多平面重建,调整冠状、轴向与矢状轴,选取通过切缘和根尖的牙齿长轴的矢状截面测量上颌中切牙、侧切牙、尖牙治疗前后牙根的变化。比较治疗前后上颌中切牙、侧切牙和尖牙牙根体积和牙根吸收量的差异。结果 与治疗前比较,治疗后上颌中切牙、侧切牙和尖牙的牙根体积均减小,差异有统计学意义(P<0.05)。经矫治后,患者侧切牙牙根吸收量显著大于中切牙和尖牙的牙根吸收量(Z=-2.783,P=0.013;Z=2.248,P=0.021),但中切牙与尖牙的牙根吸收量比较差异无统计学意义(Z=1.317,P=1.354)。结论 成人骨性Ⅱ类高角错(牙合)畸形在正畸代偿治疗过程中,上颌前牙牙根均发生了较为明显的吸收,其中侧切牙最显著,中切牙次之,尖牙较少,在临床治疗中需要引起足够重视。

Cone beam computed tomography quantitative analysis of root resorption of maxillary anterior teeth before and after compensatory treatment for skeletal Class Ⅱ high angle malocclusion in adults

[Abstract] Objective To quantitatively analyze the root resorption of maxillary central incisor, lateral incisor and cuspid teeth in adult patients with skeletal Class Ⅱ high angle malocclusion before and after compensatory orthodontic treatment using the cone beam computed tomography(CBCT) three-dimensional reconstruction technique matching Mimics 20.0 software. Methods Thirty adult cases of skeletal Class Ⅱ high angle malocclusion treated with orthodontic compensation in the Department of Orthodontics, Guiyang Stomatological Hospital from November 2016 to November 2018 were selected. Multiplanar reconstruction of CBCT data was performed by using Mimics 20.0 software to adjust coronal, axial and sagittal axises. The sagittal section with the long axis of the incisor edge and root tip was selected to measure the root changes of maxillary central incisor, lateral incisor and canine before and after treatment. The differences in root volume and root resorption of maxillary central incisor, lateral incisor and canine before and after treatment were compared. Results Compared with those before treatment, the root volumes of maxillary central incisors, lateral incisors and canines were reduced after treatment, and the differences were statistically significant(P<0.05). After the orthodontic treatment, the root resorption of lateral incisors was significantly greater than that of central incisors and canines in the patients(Z=-2.783, P=0.013; Z=2.248, P=0.021), but there was no statistically significant difference between central incisors and canines in root resorption(Z=1.317, P=1.354). Conclusion In the process of orthodontic compensation treatment for adult skeletal Class Ⅱ high angle malocclusion, the root of maxillary anterior teeth has obvious absorption. Among maxillary anterior teeth, lateral incisors have the most significant absorption, followed by central incisors, and canines have the least absorption, to which enough attention should be paid in clinical treatment.
