活动性肺结核患者血清CXCL10及CXCR3表达与CD27+CD4+ T细胞比例的相关性分析

[摘要] 目的 分析活动性肺结核患者血清CXC趋化因子配体10(CXCL10)及其受体CXC趋化因子受体3(CXCR3)与外周血单个核细胞(PBMC)中CD27+CD4+ T细胞比例的相关性。方法 选择2019年4月至2021年4月河南省胸科医院收治的活动性肺结核患者100例(肺结核组),另选择同期健康体检者96名作为对照组。应用酶联免疫吸附法(ELISA)检测受试者血清CXCL10、CXCR3、干扰素-γ(IFN-γ)、肿瘤坏死因子-α(TNF-α)的表达水平。应用流式细胞仪检测PBMC中CD27+CD4+ T细胞水平。采用Pearson相关性分析探讨血清CXCL10、CXCR3水平与PBMC中CD27+CD4+ T细胞水平以及血清IFN-γ、TNF-α水平的相关性。采用受试者工作特征(ROC)曲线分析血清CXCL10、CXCR3对活动性肺结核的诊断效能。结果 肺结核组患者血清CXCL10、CXCR3、IFN-γ、TNF-α水平均高于对照组,差异有统计学意义(P<0.05)。肺结核组PBMC的CD27+CD4+ T细胞比例低于对照组,差异有统计学意义[(22.65±2.53)% vs (41.57±5.32)%;t=31.997,P=0.000]。Pearson相关性分结果显示,活动性肺结核患者血清CXCL10与CXCR3、IFN-γ、TNF-α水平呈正相关(P<0.05),与PBMC中CD27+CD4+ T细胞比例呈负相关(P<0.05)。血清CXCR3水平与血清IFN-γ、TNF-α水平呈正相关(P<0.05),与PBMC中CD27+CD4+ T细胞比例呈负相关(P<0.05)。ROC曲线分析结果显示,血清CXCL10、CXCR3均具有诊断活动性肺结核的应用价值(AUC=0.825,AUC=0.820;P<0.05),联合两个指标可进一步提高诊断活动性肺结核的效能(AUC=0.938,P=0.000),灵敏度和特异度分别为94.00%和91.70%。结论 活动性肺结核患者血清CXCL10及CXCR3水平上调,可能与CD27+CD4+ T细胞比例低有关,联合两者检测可辅助诊断活动性肺结核。

Analysis on correlation of serum CXCL10 and CXCR3 expressions with the proportion of CD27+CD4+ T cells in patients with active pulmonary tuberculosis

Abstract:
[Abstract] Objective To analyze the correlation of serum CXC chemokine ligand 10(CXCL10) and its receptor chemokine(C-X-C motif) receptor 3(CXCR3) with the proportion of CD27+CD4+ T cells in peripheral blood mononuclear cells(PBMC) in patients with active pulmonary tuberculosis. Methods One hundred patients with active pulmonary tuberculosis who were admitted to Henan Chest Hospital from April 2019 to April 2021 were selected as the pulmonary tuberculosis group, and 96 healthy people who underwent physical examination during the same period were selected as the control group. The expression levels of serum CXCL10, CXCR3, interferon-γ(IFN-γ) and tumor necrosis factor-α(TNF-α) in the subjects were detected by using enzyme-linked immunosorbent assay(ELISA). The level of CD27+CD4+ T cells in PBMC was detected by using flow cytometry. Pearson correlation analysis was used to investigate the correlation between serum CXCL10 and CXCR3 levels and CD27+CD4+ T cell level in PBMC, serum IFN-γ level and TNF-α level. The receiver operating characteristic(ROC) curve was used to analyze the diagnostic efficiency of serum CXCL10 and CXCR3 in active pulmonary tuberculosis. Results The serum levels of CXCL10, CXCR3, IFN-γ and TNF-α in the pulmonary tuberculosis group were higher than those in the control group, and the differences were statistically significant(P<0.05). The proportion of CD27+CD4+ T cells in PBMC in the pulmonary tuberculosis group was lower than that in the control group, and the difference was statistically significant[(22.65±2.53)% vs (41.57±5.32)%; t=31.997, P=0.000]. The results of Pearson correlation analysis showed that the serum CXCL10 of the patients with active pulmonary tuberculosis was positively correlated with the levels of CXCR3, IFN-γ and TNF-α(P<0.05), and was negatively correlated with the proportion of CD27+CD4+ T cells in PBMC(P<0.05). The level of serum CXCR3 was positively correlated with the levels of serum IFN-γ and TNF-α(P<0.05), and was negatively correlated with the proportion of CD27+CD4+ T cells in PBMC(P<0.05). The results of ROC curve analysis showed that both serum CXCL10 and CXCR3 had application value in the diagnosis of active pulmonary tuberculosis(AUC=0.825, AUC=0.820; P<0.05), and the combination of the two indicators could further improve the diagnostic efficiency of active pulmonary tuberculosis(AUC=0.938, P=0.000), and the sensitivity and specificity were 94.00% and 91.70%, respectively. Conclusion The levels of serum CXCL10 and CXCR3 are up-regulated in patients with active pulmonary tuberculosis, which may be related to the low proportion of CD27+CD4+ T cells. The combined detection of the two indicators can assist in the diagnosis of active pulmonary tuberculosis.
