超声压力-应变环技术评估结缔组织病合并间质性肺疾病患者左心室心肌做功的应用价值研究

摘要:
[摘要] 目的 研究超声压力-应变环(PSL)技术评估结缔组织病合并间质性肺疾病(CTD-ILD)患者左心室心肌做功的应用价值。方法 选取2018年2月至2019年12月广西壮族自治区人民医院收治的CTD-ILD患者28例(CTD-ILD组),同期招募性别、年龄匹配的体检健康者28名作为对照组。所有受试者均完成血压测量并进行常规超声心动图检查,采集相关动态图像,应用EchoPAC软件对所采集图像进行分析。比较两组左心室心肌做功参数,包括整体做功指数(GWI)、整体有效做功(GCW)、整体无效做功(GWW)、整体做功效率(GWE),并对心肌做功参数与左室应变参数进行相关性分析。结果 与对照组相比,CTD-ILD组GWW升高,左心室整体长轴应变(GLS)绝对值、GWI、GCW及GWE均减低,差异均有统计学意义(P<0.05)。GWI、GWE与GLS绝对值呈正相关(P<0.05),GWW与GLS绝对值呈负相关(P<0.05)。结论 PSL技术可定量评估CTD-ILD患者左心室心肌做功状态,为临床提供更多参考信息。

Abstract:
[Abstract] Objective To study the application value of ultrasonic pressure-strain loop(PSL) technique in evaluating left ventricular myocardial work in patients with connective tissue disease complicated with interstitial lung disease(CTD-ILD). Methods Twenty-eight CTD-ILD patients who were admitted to the People′s Hospital of Guangxi Zhuang Autonomous Region from February 2018 to December 2019(CTD-ILD group) and twenty-eight gender and age-matched healthy individuals receiving health checkup were recruited as the control group during the same period. Blood pressure measurements and conventional echocardiography were completed for all the selected subjects. The relevant dynamic images were collected and analyzed by using EchoPAC software and the myocardial work parameters of the left ventricles were compared between the two groups, including global work index(GWI), global constructive work(GCW), global wasted work(GWW) and global work efficiency(GWE). The correlation between the parameters of myocardial work and left ventricular strain was analyzed. Results Compared with those in the control group, GWW in the CTD-ILD group increased(P<0.05), and the absolute value of global longitudinal strain(GLS), GWI, GCW and GWE decreased, and the differences were significant(P<0.05). GWI and GWE were positively correlated with the absolute value of GLS(P<0.05), and GWW was negatively correlated with the absolute value of GLS(P<0.05). Conclusion PSL can quantitatively evaluate the left ventricular myocardial work status in patients with CTD-ILD, which provides more reference information for clinical practice.
