老年CAP患者血清D-二聚体 CRP和PCT水平变化及其与疾病严重程度的相关性研究

摘要:
[摘要] 目的 探讨老年社区获得性肺炎(CAP)患者血清D-二聚体、C反应蛋白(CRP)和降钙素原(PCT)水平变化与CAP疾病严重程度的相关性。方法 选取2016-01~2017-02该院收治126例老年CAP患者为研究对象,依照患者肺炎严重指数、肺炎标准评定、患者预后等分组,另外选取同期健康体检人员30名为对照组,测定患者D-二聚体、CRP和PCT水平变化,比较不同组别患者指标差异。结果 中危组和高危组患者D-二聚体、PCT和CRP水平均显著高于低危组(P<0.05),高危组患者D-二聚体、PCT和CRP水平显著高于其他两组(P<0.05)。重症肺炎组D-二聚体、PCT和CRP水平显著高于非重症肺炎组(P<0.01),死亡组患者D-二聚体、PCT和CRP水平显著高于生存组(P<0.01)。D-二聚体与预后严重程度存在正相关(r=0.539,P=0.005),CRP与预后严重程度存在正相关(r=0.329,P=0.008),PCT与预后严重程度存在正相关(r=0.729,P=0.003)。结论 老年CAP患者血清D-二聚体、CRP、PCT与严重程度存在明显相关性,PCT和D-二聚体可以作为预测CAP预后指标。

Abstract:
[Abstract] Objective To investigate the correlation between the levels of plasma D-dimer, C-reactive protein(CRP) and calcitonin(PCT) and severity in elderly patients with community acquired pneumonia(CAP).Methods 126 elderly patients with CAP were collected in our hospital from January 2016 to February 2017 and their clinical data were retrospectively analyzed. The patients were divided into low-risk group(n=54), intermediate-risk group(n=45) and high risk group(n=27). Other 30 healthy people receiving physical examination were taken as the control group(n=30). The levels of pasmn D-dimer, CRP and PCT were detected and compared among the groups.Results The levels of plasma D-dimer, CRP and PCT in the intermediate-risk group and the high-risk group were significantly higher than those in the low-risk group(P<0.05). The levels of plasina D-dimer, PCT and CRP in the high-risk group were significantly higher than those in the two other groups(P<0.05). The levels of plasma D-dimer, PCT and CRP in the patients with severe CAP were significantly higher than those in the patients without severe CAP(P<0.01). The levels of D-dimer, PCT and CRP in the dead patients were significantly higher than those in the survival patients (P<0.01). The severity of CAP was positively correlated with the levels of plasma D-dimer(r=0.539, P=0.005), CRP(r=0.329, P=0.008) and PCT(r=0.729, P=0.003).Conclusion The levels of plasma D-dimer, CRP and PCT are correlated with the severity of CAP in elderly patients-PCT and D-dimer which can be a predicting index for prognosis.
