| ［摘要］ 目的 分析硬膜外镇痛分娩对脐动、静脉血气值的影响。方法 选择健康单胎初产妇61例，依据产妇镇痛自愿原则分为镇痛组36例，对照组25例。镇痛组采用患者自控硬膜外镇痛，在宫口开至3 cm时，L2-3间隙置入硬膜外导管连接镇痛泵。对照组产妇自愿放弃镇痛措施。在胎儿娩出后即可取脐动、静脉血各2 ml行血气分析。结果 两组新生儿脐动、静脉血气分析指标均在正常范围，镇痛组脐动脉乳酸Lac明显低于对照组，差异有统计学意义（P<0.05）。镇痛总用药量与脐动脉乳酸Lac呈负相关性，相关系数-0.553。结论 硬膜外镇痛分娩对脐动、静脉血血气值无明显影响，同时可降低脐动脉血乳酸浓度，有利于新生儿。 |

| ［Abstract］ Objective To analyze the effect of epidural labor analgesia on umbilical artery and vein blood gas values.Methods 61 healthy singleton primiparas were chosen and divided into analgesia group and control group according to voluntary principle. The analgesia group(n=36) received epidural analgesia. When the uterine ostium was open to 3 cm, L2-3 lumbar vertebral space was indwelled epidural catheter with analgesia pump. The control group(n=25) voluntarily gave up any analgesia for labor. 2 ml of umbilical artery and vein blood was drawn respectively for blood gas assay after delivery.Results The results of umbilical artery and vein blood gas analysis were within the normal ranges in both of the two groups. The level of lactic acid of umbilical artery in the analgesia group was significantly higher than that in the control group(P<0.05). The analgesic total dose showed a negative correlation with the level of lactic acid of umbilical artery(r=-0.553).Conclusion Epidural labor analgesia has no significant effect on umbilical artery and vein blood gas values, but can reduce the level of lactic acid of umbilical artery, which is beneficial to the neonates. |
