高通量血液透析联合茶多酚对糖尿病肾病维持性血液透析患者氧化应激及营养状况的影响

摘要:
[摘要] 目的 探讨高通量血液透析(HFHD)联合茶多酚对糖尿病肾病(DN)维持性血液透析(MHD)患者氧化应激及营养状况的影响。方法 选取接受MHD治疗的DN患者61例,按随机数字表法分为联合组(n=30)和对照组(n=31)。对照组先给予3个月的HFHD治疗,之后3个月采用血液透析(HD)治疗。联合组在行HFHD或HD治疗期间同时给予茶多酚片剂口服。比较两组治疗前、治疗3个月、6个月时的氧化应激指标[血丙二醛(MDA)、超氧化物歧化酶(SOD)、总抗氧化能力(T-AOC)]及营养指标(血红蛋白、白蛋白及握力)的变化。结果 治疗3个月、6个月时两组的MDA水平先降低随后升高,而SOD、T-AOC水平则先升高随后降低。联合组治疗3个月、6个月时的MDA水平明显低于对照组,SOD、T-AOC水平明显高于对照组(P<0.05)。治疗3个月、6个月时两组的血红蛋白、白蛋白水平及握力先升高随后降低。联合组治疗3个月、6个月时血红蛋白、白蛋白水平及握力明显高于对照组(P<0.05)。结论 HFHD联合茶多酚可有效改善DN的MHD患者的氧化应激状态,改善其营养状态,疗效比单一的HFHD或HD更佳,可有效提高DN的MHD患者生存质量。

Abstract:
[Abstract] Objective To investigate the effects of high flux hemodialysis(HFHD) combined with tea polyphenols on oxidative stress and nutritional status in diabetic nephropathy(DN) patients with maintenance hemodialysis(MHD).Methods 61 patients with DN who received MHD treatment were collected and were divided into the combination group(n=30) and the control group(n=31) according to the random number table. The control group was treated with HFHD for 3 months and then treated with HD for the next 3 months. The combination group was treated with oral tea polyphenols tablets during the same period when the paients were treated with HFHD and HD. The changes of oxidative stress indexes(blood MDA, SOD, T-AOC) and nutritional indexes(hemoglobin, albumin and grip strength) were compared between the two groups before and 3, 6 months after treatment.Results The levels of MDA decreased at the end of 3 months after treatment and increased at the end of 6 months after treatment.The levels of SOD and T-AOC increased firstly and then decreased 3, 6 months after treatment.The levels of MDA in the combination group were significantly lower than those in the control group 3 and 6 months after treatment. The levels of SOD and T-AOC were significantly higher than those in the control group(P<0.05). The levels of hemoglobin and albumin and handgrip strength increased firstly and then decreased in the two groups 3 and 6 months after treatment. The levels of hemoglobin and albumin and handgrip strength in the combination group were significantly higher than those in the control group 3 and 6 months after treatment(P<0.05).Conclusion HFHD combined with tea polyphenols can effectively improve the oxidative stress state of MHD patients with DN, improve their nutritional status, whose clinical effect is better than that of single HFHD or HD.
