有氧运动联合熊去氧胆酸治疗非酒精性脂肪性肝病的临床疗效观察

摘要:
[摘要] 目的 探讨有氧运动联合熊去氧胆酸治疗非酒精性脂肪性肝病(NAFLD)的临床疗效。方法 选取2014-04~2017-06该院诊治的NAFLD患者135例,按随机数字表法分为三组,分别为有氧运动组、熊去氧胆酸组和有氧运动联合熊去氧胆酸组(联合组),每组45例。三组患者在门诊接受相同的基础治疗。在此基础上,有氧运动组和熊去氧胆酸组分别给予有氧运动和熊去氧胆酸胶囊250 mg/次,3次/d,治疗3个月。联合组在接受有氧运动治疗的基础上同时接受熊去氧胆酸胶囊药物治疗,疗程为3个月。另选取同期健康体检者45名为对照组。观察比较三组干预前后身体形态[腰臀比(WHR)、体重指数(BMI)]变化,三组NAFLD患者干预前后肝功能、血脂及脂肪肝程度的变化。结果 干预后,三组NAFLD患者的WHR指数均较干预前降低,差异有统计学意义(P<0.05);而各组BMI指数干预前后虽有变化,但无论与干预前还是干预后各组间相比,差异均无统计学意义(P>0.05);三组患者治疗后,临床总有效率分别为44.44%、53.33%和77.78%,三组临床疗效比较差异有统计学意义(P<0.01);三组NAFLD患者丙氨酸转氨酶(ALT)、天门冬氨酸转氨酶(AST)和γ-谷氨酰转移酶(γ-GT)等肝功能指标均较治疗前降低,差异有统计学意义(P<0.05);三组患者甘油三酯(TG)、总胆固醇(TC)及低密度脂蛋白胆固醇(LDL-C)水平较治疗前降低,而高密度脂蛋白胆固醇(HDL-C)较治疗前升高,差异有统计学意义(P<0.05);三组NAFLD患者脂肪肝恢复正常率分别为8.89%、22.22%和33.33%,差异有统计学意义(P<0.05)。结论 有氧运动联合熊去氧胆酸治疗NAFLD的临床疗效优于单用有氧运动或熊去氧胆酸,能有效提高患者的肝功能,降低血脂水平,改善患者的生活质量。

Abstract:
[Abstract] Objective To investigate the clinical efficacy of aerobic exercise combined with ursodeoxycholic acid in treatment of non-alcoholic fatty liver disease(NAFLD).Methods 135 patients with NAFLD in our hospital from April 2014 to June 2017 were randomly divided into 3 groups: aerobic exercise group, ursodeoxycholic acid group and aerobic exercise combined with ursodeoxycholic acid group(combined group), with 45 cases in each group. The three groups received the same basic treatment in the outpatient clinic,besides the aerobic exercise group and the ursodeoxycholic acid group were treated with aerobic exercise plus ursodeoxycholic acid capules 250 mg for 3 months, the combined group received ursodeoxycholic acid capules plus aerobic exercise therapy for 3 months. Other 45 healthy subjects were selected as the control group. The changes of body shape(WHR, BMI) were compared among the three groups before and after intervention. The changes of liver function, blood lipid and fatty liver degree were compared among the patients with NAFLD in the three groups before and after intervention.Results After intervention, the WHR indexes in the patients with NAFLD in the three groups were significantly decreased compared with those before intervention(P<0.05).There were no significant differences in the BMI indexes among the three groups before and after intervention(P>0.05). The clinical total effective rates were 44.44%, 53.33% and 77.78% in the three group respectively(P<0.01). The levels of ALT, AST and gamma-GT in the NAFLD patients of the three groups were significantly decreased compared with those before intervention(P<0.05), while HDL-C was increased than that before treatment(P<0.05). The fatty liver recovery rates were 8.89%, 22.22% and 33.33% in the NAFLD patients of the three groups(P<0.05).Conclusion The clinical curative effect of aerobic exercise combined with ursodeoxycholic acid is more superior to that of aerobic exercise or using ursodeoxycholic acid alone in treatment of NAFLD. It can effectively improve liver function, reduce blood lipids and improve the patients′ quality of life.
