股骨近端防旋髓内钉与锁定钢板内固定治疗股骨转子间骨折临床疗效比较

摘要:
[摘要] 目的 比较股骨近端防旋髓内钉(proximal femoral nail anti-rotation,PFNA)与锁定钢板内固定(proximal femur locking plate,PFLP)治疗股骨转子间骨折的临床疗效。方法 选取2014-06~2016-08该院收治的股骨转子间骨折患者110例,按随机数字表法分为PFNA组和PFLP组,各55例。分别行PFNA和PFLP治疗。比较两组手术指标与髋关节功能。结果 两组髋关节功能相近,差异无统计学意义(P>0.05);与PFLP组比较,PFNA组切口长度较小,术中出血量和Hb丢失量较少,骨折愈合时间较短,差异有统计学意义(P<0.01)。结论 PFNA与PFLP均可作为股骨转子间骨折有效的治疗方式,尤其是PFNA具有微创手术的优点,可明显缩小切口长度,减轻创伤,缩短骨折愈合时间,值得临床应用。

Abstract:
[Abstract] Objective To compare the anti-rotation intramedullary nail of proximal femur(proximal femoral nail anti-rotation, PFNA) and locking plate fixation (proximal femur locking plate, PFLP) in treatment of femoral intertrochanteric fractures.Methods From June 2014 to August 2016, 110 patients with femoral intertrochanteric fractures were collected in our hospital and were divided into two groups, with 55 cases in each group. The PFNA group was treated with PFNA and the PFLP group was treated with PFLP. The surgical indicators and hip function were compared between the two groups.Results There was no significant difference in the hip function between the two groups(P>0.05). Compared with those in the PFLP group,the incision length was shorter, the intraoperative blood loss and loss of Hb were less and the healing time was shorter in the PFNA group(P<0.01).Conclusion Both of PFNA and PFLP are an effective treatment of intertrochanteric fractures. PFNA is minimally invasive surgery, and can significantly reduce the incision length and trauma, and alleviate the pain.
