超声双多普勒同时检测胎儿两条脐动脉血流变化相关性

摘要:
[摘要] 目的 探讨超声双多普勒检测胎儿脐动脉优势,探寻胎儿两条脐动脉血流变化相关性。方法 在常规扫查胎儿脐带基础上,应用先进超声双多普勒测量技术,对120例孕28~42周孕妇胎儿脐动脉(a、b组)及其血流、S/D比值、阻力指数(RI)进行检测。结果 随着孕龄的增加正常胎儿两条脐动脉的S/D比值、RI均呈下降趋势,且两条脐动脉在同一时刻S/D比值、RI差异无统计学意义(P>0.05),呈正相关性。结论 超声双多普勒测量技术较传统单一多普勒测量更有优势,为临床上实时掌握胎儿两条脐动脉血流变化(正常胎儿两条脐动脉发育、血流变化基本一致),发现脐带疾病,预测胎儿宫内窘迫情况,指导分娩方式的选择,避免胎儿宫内缺氧、死亡具有重要临床意义。

Abstract:
[Abstract] Objective To investigate the advantages of dual-band doppler ultrasonography in measuring the umbilical arteries in fetuses and to study the correlation of blood flow variation of two umbilical arteries in fetuses. Methods The umbilical cords of the fetuses in two hundred pregnant women at 28~42 weeks′ gestation were scanned routinely with the dual-band doppler ultrasonography. The blood flow of the two umbilical arteries in the fetuses were displayed simultaneously (group a and b).The S/D ratio and resistance index (RI) were measured at the same time. The real time blood flow changes of the two umbilical arteries in the fetuses were showed and the correlation was analyzed. Results The S/D ratio and resistance index of the two umbilical arteries in the normal fetuses have the downtrend with the increase in gestational age.There were no significant differences in the S/D ratio and resistance index between the two umbilical arteries in the normal fetuses. Conclusion The dual-band doppler ultrasonography has more advantages than the traditional single-band doppler ultrasonography in the real-time display of the blood flow of two umbilical arteries in fetuses which can find the umbilical cord diseases earlier and predict the fetal distress better. It also helps to choose the correct mode of delivery and avoid the fetus intrauterine anoxia.
