| ［摘要］ 目的 探讨厄贝沙坦联合硝苯地平治疗原发性高血压的临床疗效及对患者左心室舒张功能和生活质量的改善情况。方法 选取该院2014-02～2017-02的112例原发性高血压患者，随机分为观察组（59例）和对照组（53例）。对照组采用厄贝沙坦加量治疗，观察组采用厄贝沙坦常规治疗剂量联合硝苯地平治疗，连续治疗5个月。观察两组患者的血压变化、心功能指标变化、疗效及不良反应发生率情况。结果 治疗后观察组的收缩压（SBP）和舒张压（DBP）下降程度显著大于对照组（P<0.05），两组患者的E/A比值显著增高，其他心功能指标均显著降低，观察组的变化程度都显著大于对照组（P<0.05）。观察组显效54例，有效3例，无效2例；对照组显效25例，有效15例，无效13例；观察组的临床疗效显著优于对照组（P<0.01）。观察组的不良反应发生率（8.5%）低于对照组（28.3%），差异有统计学意义（P<0.05）。结论 采用厄贝沙坦联合硝苯地平的治疗方法可以提高原发性高血压患者的治疗效果，并且可以显著改善患者的左心室舒张功能，提高生活质量。 |

| ［Abstract］ Objective To investigate the effects of irbesartan combined with nifedipine on the treatment outcome, left ventricular diastolic function and quality of life in patients with essential hypertension. Methods A total of 112 patients with essential hypertension were selected in our hospital from February 2014 to February 2017 and were randomly divided into the control group(n=53, treated with large dose of irbesartan) and the observation group(n=59, treated with regular dose of irbesartan plus nifedipine). Both of the two groups were treated for 5 months. The changes of blood pressure, cardiac function therapeutic effect and incidence of adverse reactions were compared between the two groups. Results After treatment, the decreases of SBP and DBP in the observation group were significantly higher than those in the control group(P<0.05). The E/A ratios were significantly increased, and the other indexes of cardiac function were significantly decreased in both of the two groups. The degree of change in the observation group was significantly higher than that in the control group(P<0.05). In the observation group, 54 cases were markedly effective, 3 cases effective and 2 cases in effective. In the control group, 25 cases were markedly effective, 15 cases effective and 13 cases ineffective. The therapeutic effect of the observation group was better than that of the control group(P<0.01). The incidence of adverse reactions in the observation group(8.5%) was currently lower than that in the control group(28.3%)(P<0.05). Conclusion Irbesartan combined with nifedipine treatment can improve the therapeutic effect for the patients with essential hypertension, and can significantly improve the patients′ left ventricular diastolic function and quality of life. |
