产前诊断先天性胆系异常及预后评估的临床研究

摘要:
[摘要] 目的 探讨产前超声对胎儿胆系异常产前诊断及预后评估的临床价值。方法 纳入2010-01~2018-01于该院行产前超声检查提示胆系异常的病例56例,对其预后进行评估,超声提示结局良好者随访至产后1~6个月。结果 31例胎儿型胆囊结石;18例胆总管囊肿;3例孤立性胆囊不显示,其中1例羊水穿刺提示为胆囊囊性纤维化;1例先天性心内膜垫缺损合并胆囊不显示;1例胆囊长大;1例内脏反位(右位心),产后持续性黄疸,发现胆道闭锁;1例腹腔巨大囊性占位,产后提示为胆总管囊肿。结论 胆系筛查目前尚未纳入超声产前筛查范畴,超声及早诊断胆系异常可评估预后,指导临床,降低引产率,提高新生儿生存率。

Clinical study on prenatal diagnosis and prognosis evaluation of congenital biliary abnormalities

Abstract:
[Abstract] Objective To explore the clinical value of ultrasound in prenatal diagnosis and prognosis evaluation of fetal biliary abnormalities. Methods A total of 56 cases with abnormal biliary system were diagnosed by prenatal ultrasound examination in our hospital from January 2010 to January 2018, and their prognoses were evaluated. The patients with good outcomes were followed up for 1 to 6 months after delivery. Results The ultrasound investigation revealed fetal cholelithiasis in 31 cases, and choledochal cyst in 18 cases, but 3 cases of solitary gallbladder were undetected among whom amniocentesis indicated cystic fibrosis of gallbladder in 1 case. The ultrasonic results also showed one case of congenital endocardial cushion defect combined with gallbladder failed to display, 1 case of gallbladder bulge, 1 case of situs inversus(dextrocardia) with postpartum persistent jaundice and biliary atresia, and 1 case with a large cystic mass in the abdominal cavity which was diagnosed as a choledochal cyst. Conclusion Currently, biliary screening has not been included in the scope of ultrasonic prenatal screening. Early diagnosis of biliary abnormalities by ultrasound can evaluate the prognosis, guide the clinical practice, reduce the rate of labor induction and improve the survival rate of newborns.
