中耳胆脂瘤显微手术技术的改进

摘要:
[摘要] 目的 探讨乳突上鼓室填塞及纳吸绵应用在中耳胆脂瘤手术(开放式鼓室成形术)中的价值。方法 选取80例单侧胆脂瘤患者,随机分为观察组(50例)和对照组(30例),观察组在开放式手术后行乳突上鼓室填塞及纳吸绵应用,对照组行单纯开放式鼓室成形术。对比两组术后渗液量、堵塞感、纯音听阈和鼓室图的差异。结果 观察组在术后渗液量评分≤2分的人数比例和气骨导差≤20 dB的人数比例大于对照组,差异有统计学意义(P<0.05)。两组在术后堵塞感评分≤2分和出现B型曲线的发生情况比较差异无统计学意义(P>0.05)。结论 开放式鼓室成形术后乳突上鼓室填塞及纳吸绵应用可改善手术效果。

Abstract:
[Abstract] Objective To explore the value of mastoid and attic obliteration and NasoPore packing in surgical treatment of middle ear cholesteatoma(open tympanoplasty). Methods Eighty cases of unilateral middle ear cholesteatoma were randomly divided into observation group and control group. The observation group(n=50) underwent mastoid and attic obliteration and NasoPore packing after open tympanoplasty, and the control group(n=30) received open tympanoplasty alone. The differences of ear discharge volume, ear plugging sensation, pure tone threshold and tympanogram after surgery were compared between the two groups. Results The proportion of the patients in the observation group with the postoperative discharge scores ≤2 points and the proportion of the patients with the air-bone gap ≤2 dB were significantly larger than those in the control group(P<0.05). There were no significant differences in the postoperative ear plugging sensation scores ≤2 points and the occurrence of B-shaped curve between the two groups(P>0.05). Conclusion Mastoid and attic obliteration and NasoPore packing can improve the prognosis of the surgical treatment of middle ear cholesteatoma after open tympanoplasty.
