| ［摘要］ 目的 比较不同缝线缝合法对腹腔镜子宫肌瘤切除术（LM）术中、术后及妊娠结局的影响。方法 选择2013-01～2018-03于该院接受LM治疗的子宫肌壁间肌瘤患者174例。根据手术不同缝线、缝合法将其分为薇乔线褥式法组（34例）、薇乔线棒球法组（34例）、V-Loc线褥式法组（55例）和V-Loc线棒球法组（51例）。比较四组术中出血量、手术时间，术后血红蛋白（Hb）下降程度、肌瘤残留率和妊娠情况。结果 四组手术时间、术中出血量、Hb下降量方面比较差异有统计学意义（P<0.05）。在随访期间，受孕成功52例，共59次，妊娠率为29.89%(52/174)。其中5例首次妊娠流产后再次足月妊娠1次，1例2次妊娠流产后足月妊娠1次。四组妊娠率及妊娠间隔时间比较差异无统计学意义（P>0.05）。四组无一例发生术中、术后并发症，术后3个月复查B超均无肌瘤残留。结论 采用V-Loc线棒球法缝合可缩短手术时间，减少术中术后出血量。不同缝线缝合法可对患者LM后妊娠结局有一定影响。 |

| ［Abstract］ Objective To compare the effects of different sutures and suture methods on laparoscopic myomectomy(LM) during and after operation and pregnancy outcomes. Methods One hundred and seventy-four patients with uterine myometrial fibroids receiving LM treatment in our hospital from January 2013 to March 2018 were selected. According to different sutures and suture methods, the patients were divided into the common absorbable pad suture group(n=34), the common absorbable baseball suture group(n=34), the V-Loc pad suture group(n=55) and the V-Loc baseball suture group(n=51). The intraoperative blood loss, the operation time, the degree of hemoglobin(Hb) decrease after operation, the rate of residual myoma and pregnancy were compared among the four groups. Results There were statistically significant differences in the operation time, the intraoperative blood loss and the degree of Hb decrease among the four groups(P<0.05). During the follow-up period, 52 cases were pregnant successfully, with a total of 59 times of pregnancy, and the pregnancy rate was 29.89%(52/174). Among them, 5 cases had a full-term pregnancy once again after the first pregnancy abortion; 1 case had a full-term pregnancy after 2 times of miscarriage. There were no significant differences in the pregnancy rate and pregnancy interval among the four groups(P>0.05). There were no complications during and after operation in the four groups. No residual fibroids were found in B-ultrasound examination 3 months after operation. Conclusion Using V-Loc baseball suture can shorten the operation time and reduce the amount of intraoperative and postoperative bleeding. Different sutures and suture methods have certain impacts on the patients′ pregnancy outcomes after LM. |
