注意缺陷多动障碍不同亚型儿童行为问题和社会生活能力分析

摘要:
[摘要] 目的 分析注意缺陷多动障碍(ADHD)不同亚型儿童的行为问题和社会生活能力的差异,为临床治疗提供依据。方法 根据DSM-Ⅴ诊断标准纳入225例ADHD儿童,其中混合型(ADHD-C)103例,注意缺陷型(ADHD-I)90例,多动/冲动型(ADHD-HI)32例,纳入同期正常儿童112名,分别采用Conners父母症状问卷(PSQ)、婴儿-初中学生社会生活能力量表(S-M)进行评估,比较ADHD儿童与正常儿童、不同亚型ADHD儿童的行为问题及社会生活能力差异。结果 ADHD儿童行为问题的6个因子(品行问题、学习问题、心身问题、冲动-多动指数、焦虑及多动指数)评分均高于正常对照组儿童(P<0.01);而各项社会生活能力均低于正常对照组儿童(P<0.05)。Spearman相关性分析显示ADHD儿童的品行问题、学习问题、冲动-多动、多动指数得分越高,社会生活能力总分越低(P<0.01)。ADHD-C儿童品行问题、学习问题、冲动-多动指数、多动指数分数较其他两亚型高,ADHD-I儿童学习问题分数高于ADHD-HI儿童(P<0.05),冲动-多动指数分数低于ADHD-HI儿童。相较于ADHD-I及ADHD-HI儿童,ADHD-C儿童社会生活能力总分、独立生活、作业、交往、集体活动、自我管理能力较差(P<0.05)。结论 ADHD儿童行为问题较正常儿童更为明显,社会功能损害也更严重,行为问题越严重,其社会生活能力越差。不同亚型ADHD儿童临床特点不同,其造成的行为问题及功能损害也不一致。

Abstract:
[Abstract] Objective To analyze the differences in behavior problems and social living ability of children with different subtypes of attention deficit hyperactivity disorder(ADHD), so as to provide basis for clinical treatment. Methods According to the Diagnostic and Statistical Manual of Mental Disorders(DSM-Ⅴ) diagnostic criteria, 225 children with ADHD were enrolled, including 103 cases of combined type of ADHD(ADHD-C), 90 cases of predominantly inattentive type of ADHD(ADHD-I), and 32 cases of predominantly hyperactive/impulsive type of ADHD(ADHD-HI), and 112 normal children were included in the same period. Conners Parent Symptom Questionnaire(PSQ) and Normal Development of Social Skills from Infant to Junior High School Children(S-M) were respectively used to evaluate and compare the differences in behavior problems and social living ability between ADHD children and normal children, and among different subtypes of ADHD children. Results The scores of six factors of behavioral problems in ADHD children(conduct problems, learning problems, psychosomatic problems, Impulse-hyperactivity Index, anxiety and Hyperactivity Index) were higher than those in the children in the normal control group(P<0.01), while the various social living abilities of ADHD children were lower than those of the children in the normal control group(P<0.05). Spearman correlation analysis showed that the higher the scores of conduct problems, learning problems, Impulse-hyperactivity Index, and Hyperactivity Index in ADHD children, the lower the total score of social living ability(P<0.01). The scores of conduct problems, learning problems, Impulse-hyperactivity Index, and Hyperactivity Index of ADHD-C children were higher than those of the other two subtypes children, and the scores of learning problems of ADHD-I children were higher than those of ADHD-HI children(P<0.05), and the Impule-hyperactivity Index scores of ADHD-I children was lower than those of ADHD-HI children. Compared with ADHD-I children and ADHD-HI children, ADHD-C children had the worse total score of social living ability, independent living ability, homework, communication, group activity and self-management ability(P<0.05). Conclusion Children with ADHD have more obvious behavior problems than normal children, and the social functional impairment of the ADHD children is more serious, and the more serious the behavior problems, the worse their social living ability. The clinical characteristics of children with different subtypes of ADHD are different, and the behavior problems and functional impairment caused by the different subtypes of ADHD are also inconsistent.
