妊娠期肝内胆汁淤积症患者血清25-羟基维生素D3 甘胆酸和总胆汁酸水平与不良妊娠结局的关联性研究

摘要:
[摘要] 目的 探讨妊娠期肝内胆汁淤积症(ICP)患者体内甘胆酸(CG)、总胆汁酸(TBA)、血清25-羟基维生素D3[25(OH)Vit D3]与不良妊娠结局的关联性。方法 收集2019年1月至2020年4月在该院诊治的ICP患者153例,分为轻度ICP组(114例)和重度ICP组(39例),纳入同期产检的健康孕妇153名作为健康对照组。采集血样后行CG、TBA、肝酶和25(OH)Vit D3检测,随访妊娠结局。结果 ICP孕妇血清CG、TBA和肝酶水平显著高于健康孕妇,且随着疾病严重程度增加而增加,血清25(OH)Vit D3水平随着疾病严重程度增加而降低,差异有统计学意义(P<0.05)。孕妇的CG和TBA水平与25(OH)Vit D3水平呈负相关(P<0.05)。ICP孕产妇分娩孕周小于健康对照组,且ICP组不良妊娠结局发生率高于健康对照组,差异有统计学意义(P<0.05)。结论 ICP孕妇CG、TBA和肝酶水平高于健康孕妇,25(OH)Vit D3水平低于健康孕妇。高水平的CG、TBA和低水平的25(OH)Vit D3对围产期不良妊娠结局产生不利的影响。

Abstract:
[Abstract] Objective To investigate the association of cholyglycine(CG), total bile acid(TBA) and serum 25-hydroxyvitamin D3[25(OH)Vit D3] with the adverse pregnancy outcomes in patients with intrahepatic cholestasis of pregnancy(ICP). Methods From January 2019 to April 2020, a total of 153 patients who were diagnosed with ICP and treated in Citic Huizhou Hospital were collected and divided into mild ICP group(114 cases) and severe ICP group(39 cases). One hundred and fifty-three healthy pregnant women who received antepartum examination in the same period were included as healthy control group. After blood samples were collected, CG, TBA, liver enzymes and 25(OH)Vit D3 were detected, and the patients were followed up for pregnancy outcomes. Results The serum levels of CG, TBA and liver enzymes in the ICP pregnant women were significantly higher than those in the healthy pregnant women, and increased with the increase of disease severity, and the serum level of 25(OH)Vit D3 decreased with the increase of disease severity, and the differences were statistically significant(P<0.05). The level of 25(OH)Vit D3 was negatively correlated with CG and TBA in the pregnant women(P<0.05). The gestational weeks of the ICP pregnant women were less than those of the pregnant women in the healthy control group, and the incidence of adverse pregnancy outcomes in the ICP group was higher than that in the healthy control group, and the differences were statistically significant(P<0.05). Conclusion The levels of CG, TBA and liver enzymes in ICP pregnant women are higher than those in healthy pregnant women. The levels of 25(OH)Vit D3 in ICP pregnant women are lower than those in healthy pregnant women. High levels of CG, TBA and low levels of 25(OH)Vit D3 have adverse effects on perinatal pregnant outcomes.
