视网膜激光光凝治疗对近视患者眼调节功能影响的观察研究

摘要:
[摘要] 目的 观察视网膜激光光凝治疗对近视患者眼调节功能的影响。方法 招募2022年7月至2023年7月在惠州市第三人民医院行视网膜激光光凝治疗患者40例,分别于激光光凝治疗前及治疗后1周、2周、3周、1个月检查患者的远视力、近视力及眼调节功能[包括调节幅度(AMP)、双眼正相对调节(PRA)及负相对调节(NRA)和调节反应(BCC)]。结果 视网膜激光光凝治疗前后患者的远视力、近视力及BCC无显著变化。AMP和NRA在治疗后1周显著下降(P<0.05),随后逐渐提高,但在治疗后1个月,患者AMP仍未恢复到术前水平(P<0.05),而NRA在术后3周时已与术前无显著差异(P>0.05)。PRA在治疗后1周显著降低(P<0.05),随后逐渐恢复,至治疗后1个月时与治疗前无显著差异(P>0.05)。结论 视网膜激光光凝治疗后1周患者眼调节功能指标AMP、NRA、PRA降低,之后逐渐恢复。建议行视网膜激光光凝治疗的近视患者最好选择在治疗3周后再行近视矫正手术。

A study on the observation of the effect of retinal photocoagulation treatment on ocular accommodation function in patients with myopia

Abstract:
[Abstract] Objective To observe the effect of retinal photocoagulation treatment on ocular accommodation function in patients with myopia. Methods Forty patients undergoing retinal photocoagulation treatment in the Third People′s Hospital of Huizhou City from July 2022 to July 2023 were recruited. The patients′ distance visual acuity, near visual acuity and ocular accommodation function[including amplitude of accommodation(AMP), binocular positive relative accommodation(PRA), binocular negative relative accommodation(NRA) and binocular cross-cylinder(BCC)] were examined before retinal photocoagulation treatment and 1 week, 2 weeks, 3 weeks and 1 month after retinal photocoagulation treatment, respectively. Results There were no significant changes in distance visual acuity, near visual acuity and BCC before and after retinal photocoagulation treatment. AMP and NRA decreased significantly 1 week after treatment(P<0.05), and then gradually increased. However, the patients′ AMP had not returned to the preoperative level 1 month after treatment(P<0.05), and there was no significant difference between the level of NRA 3 weeks after surgery and that before surgery(P>0.05). PRA decreased significantly 1 week after treatment(P<0.05), and then recovered gradually, and there was no significant difference between the level of PRA 1 week after surgery and that before surgery(P>0.05). Conclusion After 1 week of retinal photocoagulation treatment, the indicators of AMP, NRA and PRA in the patients′ ocular accommodation function decreased, and gradually recovered thereafter. It is recommended that myopic patients receiving retinal photocoagulation treatment choose to undergo myopia correction surgery after 3 weeks of the treatment.
