内镜下蓝激光成像技术及碘染色诊断多原发食管癌的对比研究

摘要:
[摘要]目的 探讨内镜下蓝激光成像技术(blue laser imaging,BLI)与碘染色对多原发食管癌的诊断价值。方法 分析2015年6月-2018年8月在广西壮族自治区人民医院消化内镜中心经病理诊断为食管癌的437例患者,对比内镜下BLI与碘染色诊断多原发食管癌的价值。结果 经病理结果证实的多原发食管癌共47例,占所有食管癌患者的10.8%(47/437);共检出癌灶111个,分别分布于食管上段36个、中段58个、下段17个;早期食管癌39个,进展期食管癌72个。BLI诊断多原发食管癌的灵敏度、特异度、曲线下面积分别为89.4%、100.0%、0.947;碘染色诊断多原发食管癌的灵敏度、特异度、曲线下面积分别为95.7%、100.0%、0.979;两组曲线下面积相比Z=1.771,P=0.077>0.05,可以认为两组差异无统计学意义。结论 内镜下BLI与碘染色对多原发食管癌均有较高的诊断价值。

Abstract:
[Abstract]Objective To investigate the diagnostic value of endoscopic Blue Laser Imaging technology (BLI) and iodine staining for multiple primary esophageal carcinoma. Methods Analysis of 437 patients who underwent pathological diagnosis of esophageal carcinoma at the Digestive Endoscopy Center of People's Hospital of Guangxi Zhuang Autonomous Region from June to May 2018, to compare the value of BLI and iodine staining in the diagnosis of multiple primary esophageal carcinoma. Results There were 47 cases of multiple primary esophageal carcinoma confirmed by pathological results, accounting for 10.8% (47/437) of all patients with esophageal carcinoma. 111 lesions were detected, which were distributed in 36 upper, 58 middle and 17 lower segments of the esophagus, 39 early esophageal carcinoma and 72 advanced esophageal carcinoma. The sensitivity, specificity and area under curve of BLI in the diagnosis of multiple primary esophageal carcinoma were 89.4%, 100.0% and 0.947. The sensitivity, specificity and area under curve of iodine staining in the diagnosis of multiple primary esophageal carcinoma were 95.7%, 100.0% and 0.979. The area under curve of the two groups was Z = 1.771, P = 0.077 > 0.05, and there was no significant difference between the two groups. Conclusion Endoscopic iodine staining and BLI have a high diagnostic value for multiple primary esophageal carcinoma.
