目的:通过X线头影测量方法评估安氏I类错畸形早期恒牙正畸治疗拔牙与非拔牙矫治方式对上气道形态及舌骨位置的影响,进而指导临床矫治的实施。方法:收集安氏I类错畸形早期恒牙患者正畸拔牙治疗与非拔牙治疗病例各30例,对其上气道形态及舌骨位置进行测量、对比分析。结果:非拔牙组组内比较:PNS-UMPW、U-MPW、H-FH、H-VL差异有统计学意义(P<0.05),拔牙组组内比较:PNS-UMPW、SPP-SPPW、U-MPW、H-MP差异有统计学意义(P<0.05),非拔牙组与拔牙组治疗前组间比较:TB-TPPW,治疗后组间比较:PNS-UPW差异有统计学差异(P<0.05),其余测量项目无统计学差异(P>0.05)结论:安氏I类早期恒牙患者正畸拔牙矫治对上气道形态的变化无明显影响,对舌骨位置无明显影响,但矢状方向上产生向后向下的移动趋势。 |

The influence about orthodontic treatment of premolar extraction of the effects on upper airway morphology and hyoid position in Angle ClassⅠwith premature permanent occlusion |

Objective:This study was designed to observe the effects of premolar extraction on upper airway morphology and hyoid position in the Angle ClassⅠwith premature permanent occlusion by cephalometrics comparing the pretreatment and postreatment upper airway morphology and hyoid position . In order to offer a more reliable theoretical basis for making the clinical orthodontic treatment plan. Methods:This study chosed the Angle ClassⅠwith premature permanent teeth and the patients were divided into two groups according to their different therapy method,with 30 cases in each group.To observe the correlations the change of upper airway morphology and hyoid position upper airway before and after the extraction treatment. Results:Comparison before and after treatment in non-tooth extraction group:The differences in PNS-UPW、U-MPW、H-FH、H-VL measurement items were statistically significant (P < 0.05);in premolar extraction group , PNS-UPW、SPP-SPPW、U-MPW、H-MP measurement items were significantly different (P < 0.05);Comparison non-tooth extraction and premolar extraction group after treatment:there was a statistically significant difference in PNS-UPW measurement items (P < 0.05), while there was no statistically significant difference in other measurement items (P > 0.05).Conclusion:the Angle ClassⅠwith premature permanent occlision had no significant effect on the change in the upper airway morphology, it is no obvious effect on hyoid position, but produce backward on hyoid position on the sagittal direction downward movement trend.
