不插管舒适化麻醉技术对小儿日间手术安全性的临床观察

摘要:
目的：观察不同剂量芬太尼复合七氟烷不插管全麻用于小儿手术的影响。方法：选择60例手术患儿，采用随机数字表法分为S、SF1和SF2三组，每组20例；七氟烷诱导入睡，三组分别给予生理盐水、芬太尼1ug/kg和1.5ug/kg复合维持，非插管面罩紧闭给氧通气；记录患者切皮体动率、切皮后生命征及苏醒时间。结果：三组患儿苏醒时间无差异(P＞0.05)；术中血压、心率、切皮体动、七氟烷用量SF1组与SF2组显著低于S组(P＜0.05)；托下颌比率SF1组和SF2组高于S组 (P<0.05)；仅S组出现2例喉痉挛，经处理后缓解。结论：使用芬太尼复合七氟烷不插管麻醉可安全用于小儿日间手术，且提高患儿的麻醉后舒适度。

Abstract:
Objective: To observe the anesthetic effect and safety of two doses of fentanyl combined with sevoflurane for general anesthesia without intubation in children undergoing day surgery. Methods: We analyzed the case records of 60 pediatric patients in our hospital who underwent day surgery were randomly divided into three groups( 20 cases in each group);according to the doses of fentanyl, we were given 1μg/kg group （group SF1）,1.5μg/kg group （group SF2） and control group （group S） ,in which the same volume of normal saline was infused in by the vein.without endotracheal intubation and retain spontaneous breathing,the following parameters were examined: sex, age,intraoperative or postoperative complications,vital signs,recovery time and surgical method. Results: There was no significant difference between the three groups in terms of the waking hours(P 0.05),The MAP,HR, pedicle body movement and sevoflurane dosage in Group S were significantly higher than that of Group SF1 and Group SF2( P 0. 05);The mandibular support ratio of Group S significantly lower than those in Group SF1 and Group SF2( P 0. 05)，but laryngeal spasm in group S occurred Only 2 cases， which were relieved after treatment. Conclusion: fentanyl combined with sevoflurane without intubation anesthesia can be safely used in pediatric daytime surgery, and improve the comfort of children after anesthesia.
