目的 研究金黄色葡萄球菌(金葡菌)血行感染对外周血血小板数量的影响。方法 以武汉大学中南医院2014-01~2016-12收治金葡菌感染患者328例作为研究对象,其中血行感染(SAB)81例、呼吸道感染(SAR)103例、伤口局部感染(SAW)94例以及泌尿道感染(SAU)50例,血小板采用血液分析仪DxH800或LH750进行计数,金葡菌采用VITEK 2 Compact进行鉴定与药敏分析。结果 金葡菌感染发生时,SAB、SAR、SAW和SAU组外周血血小板数分别为159×109/L、177×109/L、232×109/L和(223.7±85.1)×109/L,而感染控制后,血小板数分别为202×109/L、205×109/L、260×109/L和(229.5±90.8)×109/L。统计学分析显示SAB、SAR和SAW组感染控制后血小板数显著升高,且SAB组血小板数上升率显著高于SAR和SAW组。SAB组血小板数上升率与感染控制所需时间呈正相关,而与平均血小板体积的上升率呈负相关。结论 金葡菌血行感染会导致患者外周血血小板数量的减少,且这种减少与血小板的活化和聚集有一定程度的相关性。 |

Studying the effect of Staphylococcus aureus infection on platelet counts in peripheral blood |

Abstract: |
Objective To study the effect of Staphylococcus aureus (S. aureus) blood on the number of platelets in peripheral blood. Methods The patients with Staphylococcus aureus infection in Zhongnan Hospital of Wuhan University from January 2014 to December 2016 is the subject of study, including 81 cases of blood infection (SAB), 103 cases of respiratory infection (SAR), 94 cases of local wound infection (SAW) and 50 cases of urinary tract infection (SAU). Platelet counts were performed using a blood analyzer DxH800 or LH750. Identification and drug susceptibility analysis of Staphylococcus aureus were performed using VITEK 2 Compact.Results The number of peripheral blood platelets in the SAB, SAR, SAW and SAU groups at the time of S. aureus infection were 159×109/L、177×109/L、232×109/L and (223.7±85.1)×109/L respectively, and after infection control, the platelet values were 202×109/L、205×109/L、260×109/L and (229.5±90.8)×109/L respectively.Statistical analysis showed that the number of platelets in SAB, SAR and SAW groups was significantly increased after infection control, and the rate of increase in platelet count in SAB group was significantly higher than that in SAR and SAW groups. The rate of increase in platelet count in the SAB group was positively correlated with the time required for infection control, but negatively correlated with the increase in mean platelet volume.Conclusions Staphylococcus aureus infection can lead to a decrease in the number of peripheral blood platelets, and this reduction has a certain degree of correlation with platelet aggregation and activation. |
