[摘要] 目的 探讨应用弹性髓内针治疗儿童下肢长骨骨折中的临床效果。方法 选择该院自2013-012019-12收治的34例儿童下肢长骨骨折儿童作为研究对象,按照入院时间先后及手术方式不同,将2013年1月2017年2月的17例采用切开复位钢板螺钉内固定的病例作为对照组,将2017年3月2019年12月的17例采用闭合复位小儿弹性髓内针固定的病例作为观察组。比较两组治疗结果。结果 两组病例均获定期随访,随访时间6~24个月。观察组骨折断端在术后4周拆除石膏时复查X线即可见到骨痂生成,髓内钉钉眼处均未见感染、松动、皮肤过敏等现象。观察组骨折均平均愈合时间(43.25±5.36)d,未见骨折不愈合,延迟愈合,未见骨折对线畸形,无肢体短缩,未见内固定物感染,6个月后随访所有患儿肢体活动功能均恢复正常,骨折周围关节活动无受限。对照组骨折均平均愈合时间(70.49±8.41)d,3例骨折延迟愈合,2例内固定物感染,予再次手术后骨折愈合,未见骨折对线畸形及肢体短缩畸形。观察组手术时间、出血量、住院时间及骨折愈合时间均低于对照组(P<0.05)。观察组Flynn评分高于对照组(P<0.05)。结论 弹性髓内针应用于儿童下肢长骨骨折中效果显著,值得临床应用与推广。 |

Clinical application of elastic intramedullary needle in the treatment of long bone fractures in children |

【Abstract】 Objective To explore the clinical effect of elastic intramedullary nail in the treatment of long bone fractures in children. Methods Choose 34 cases of long bone fractures of children in our hospital since January 2013 to December 2019, According to the time of admission and the type of surgery, 17 cases from January 2013 to February 2017 with open reduction and plate screw internal fixation were used as a control group, 17 cases with closed reduction and elastic intramedullary nail fixation in March 2017 December 2019 were used as the observation group. Compare the treatment results of the two groups. Results Both cases were regularly followed up for 6 to 24 months. Observation group fracture fracture ends can be seen on the X-ray when the plaster is removed 4 weeks after surgery, and there is no infection, loosening, skin allergy and other phenomena in the intramedullary nails. The average healing time of fractures in the observation group was (43.25 ± 5.36) d. No fracture nonunion, delayed union, no fracture alignment deformity, no limb shortening, no internal fixation infection.After 6 months of follow-up, all children's limb movement function returned to normal, and joint movement around the fracture was not limited. The average healing time of fractures in the control group was (70.49 ± 8.41) d. Three cases of fractures healed delayed, 2 cases of internal fixation infection, fracture healing after reoperation, no fracture alignment and short limb deformity were observed. The operation time, bleeding volume, hospitalization time and fracture healing time of the observation group were lower than those of the control group (P <0.05). The Flynn score of the observation group was higher than that of the control group (P <0.05). Conclusion The application of elastic intramedullary nail in the treatment of long bone fractures of children's lower limbs is remarkable, and it is worthy of clinical application and promotion. |
