分化型甲状腺癌颈部淋巴结转移的相关危险因素研究

摘要:
目的:分析分化型甲状腺癌(DTC)患者的超声及实验室检查结果,研究与颈部淋巴结转移相关的危险因素。方法:选取2019年1~12月于我院外科手术后病理证实为分化型甲状腺癌的患者共133例,根据是否存在颈部淋巴结转移分为转移组(78例)与无转移组(55例)。收集两组患者的临床及超声特点,应用统计学软件进行淋巴结转移的相关因素分析。结果:①单因素分析显示:两组患者性别(P =0.031)、年龄(P<0.01)、肿瘤直径(P<0.01)、原发灶内钙化(P<0.01)、血清TSH(P=0.009)、血清TGAb和TPOAb阳性率(P=0.000、0.025)差异均有统计学意义。②多因素Logistic回归分析显示:年龄、性别、肿瘤直径、钙化、血清TGAb是DTC淋巴结转移的独立危险因素。结论:分化型甲状腺癌中男性、年龄轻、肿瘤直径>1cm、有钙化、血清TSH高、血清TgAb和TPOAb为阳性的患者淋巴结转移风险更高,术前完善超声及血清学检查对指导DTC手术及预后有重要意义。

Study of risk factors related to cervical lymph node metastasis in differentiated thyroid cancer

Abstract:
Objective To analyze the ultrasound and laboratory examination results of patients with differentiated thyroid cancer (DTC),investigate risk factors that related to cervical lymph node metastasis.Methods 133 patients who are confirmed with DTC by Pathological Examinations after the surgeries were collected. Those patients were divided into two groups based on whether cervical lymph node has transferred or not (78 vs. 55). Statistics software were used to analyze related factors on the clinical and ultrasound characteristics of the two groups of patients.Results ①Univariate analysis shows: differences on gender (P=0.031), age (P<0.01),diameter of tumors (P <0.01),calcification of primary focus(P<0.01), serum TSH levels(P =0.009),positivity rate of TgAb and TPOAb(P=0.000、0.025)have statistical significances.②Multivariate logistic regression analysis shows:age,gender, diameter of tumor, calcification of primary focus,and TgAb are the independent risk factors for lymph node transfer of DTC patients. Conclusion Patients with DTC have a higher risk of lymph node metastasis in men, younger, tumor diameter>1cm, calcification, higher serum TSH, and positive serum TGAb and TPOAb.Optimizing the ultrasound and serological examination pre-operation is of important clinical significance to guide the operation and prognosis of DTC.
