| 摘要: |
| 目的观察参附注射液对脓毒症休克患者心脏舒张功能的影响及疗效观察。方法将60例感染性休克患者随机分为对照组和参附组。两组均给予规范的常规集束化治疗（液体复苏、抗感染、氧疗等）。参附组在此常规治疗基础上给予参附注射液100ml加入5%葡萄糖100ml中静滴，每日一次，疗程７ｄ。检测两组治疗前及治疗后第７ｄ血清脑钠肽（NT-proBNP）及心肌肌钙蛋白（cTnT）水平；治疗前后分别行超声心动图测定左室舒张末内径（LVEDD），左室舒张末容积（LVEDV），二尖瓣舒张早期血流峰速度（E），二尖瓣环舒张早期运动速度比值(E’），并计算E／E’，同时记录两组患者治疗前后急性生理和慢性健康评分（APACHEⅡ评分）、序贯性器官衰竭评分（SOFA）及机械通气时间，ICU入住时长。结果在感染性休克治疗1周，参附组NT-proBNP, cTnT,LVEDD、LVEDV 、E／E’较对照组下降（P＜0.05）；治疗后两组患者的APACHEⅡ评分、SOFA评分、机械通气时间，ICU入住时长相比较，参附治疗组较对照组明显降低（P＜0.05）。结论参附注射液可以改善感染性休克患者心脏舒张功能，减轻MODS的严重程度，改善疗效。 |

| Objective To observe the effect of Shenfu injection on diastolic function in patients with septic shock. Methods Sixty patients with septic shock were
randomly divided into control group and Shenfu group. Both groups were given regular routine clustering therapy (liquid resuscitation, anti-infection, oxygen therapy, etc). On the basis of this routine treatment, the Shenfu group was given 100 ml of Shenfu injection,and once daily for 7 days. Serum brain natriuretic peptide (NT-proBNP) and cardiac troponin (cTnT) levels were measured before treatment and on the 7th day after treatment. Left ventricular end-diastolic diameter (LVEDD) and left ventricular end-diastolic Volume (LVEDV)were measured by echocardiography before and after treatment. peak velocity of early mitral valve dilation (E), early diastolic velocity of mitral annulus (E'), and E/E' was calculated, APACHE II score 、SOFA score and mechanical ventilation time, ICU stay time before and after treatment were recorded in both groups. Results One week after septic shock treatment, NT-proBNP, cTnT, LVEDD, LVEDV, E/E' decreased in both groups compared with the control group; compared with the control group,The decrease of LVEDD, LVEDV, E/E' and NT-proBNP was more obvious (P<0.05). The APACHE II score ，SOFA score and Mechanical ventilation time, ICU stay time of the two groups after treatment were significantly lower than those of the control group (P<0.05). Conclusion Shenfu injection can improve the diastolic function of patients with septic shock and reduce the severity of MODS，Improve the curative effect. |
