2型糖尿病中年男性患者性激素与体脂分布的相关性

摘要:
目的 探究中年男性2型糖尿病不同体型人群性激素水平与体脂分布的相关性。方法 选择2018年5月至2019年11月期间于广西壮自治区人民医院内分泌代谢科就诊的2型糖尿病中年男性患者50例,根据体质指数(BMI)将患者分为体重正常组(BMI=18.5~23.9 Kg/m2)14例,超重组(BMI=24.0~27.9 Kg/m2)20例,肥胖组(BMI≥28.0 Kg/m2)16例。比较三组患者的体型、体脂分布、血脂和性激素,分析性激素与体脂分布的相关性。结果 超重组BMI、腰围、甘油三酯(TG)高于体重正常组;肥胖组的TG、“Android区域”脂肪比例与“Gynoid区域”脂肪比例的比值(A/G)高于体重正常组、总睾酮(TT)低于体重正常组;肥胖组的BMI、腰围、全身脂肪比例、“Android区域”脂肪比例高于体重正常组和超重组。相关性分析结果显示,TT与腰围、黄体生成素(LH)、“Android区域”脂肪比例、A/G呈负相关(r值分别为-0.318、-0.420、-0.762、-0.319,P均<0.05)。多元逐步回归分析结果显示,“Android区域”脂肪比例是TT的重要影响因素。结论 2型糖尿病中年男性患者的总睾酮水平与体脂分布密切相关;内脏脂肪蓄积是总睾酮水平下降的重要危险因素。

Abstract:
Objective To investigate the correlation between sex hormone levels and body fat distribution in middle-aged male with type 2 diabetes mellitus. Methods Fifty middle-aged male patients with type 2 diabetes mellitus were enrolled in endocrinology department of The people’s hospital of Guangxi Zhuang autonomous region from May 2018 to November 2019. According to body mass index(BMI), the patients were divided into normal weight group(BMI=18.5~23.9 Kg/m2) 14 cases, overweight group(BMI=24.0~27.9 Kg/m2) 20 cases, obesity group(BMI≥28.0 Kg/m2) 16 cases. Body size, body fat distribution and serum levels,such as sex hormones, lipid level were compared among the three groups, and the correlation between sex hormone levels and body fat distribution. Results Compared with the normal weight group, the levels of BMI, waistline, triglyceride (TG) were significantly increased in overweight group. Compared with the normal weight group, the levels of TG, adipose percentage of “android” area to “gynoid” area ratio(A/G) were significantly increased and total testosterone(TT) were significantly decreased in obesity group. Compared with the normal weight group and the overweight group simultaneously, the levels of BMI, waistline, total adipose percentage, adipose percentage of “android” area were significantly increased in obesity group. Correlation analysis showed that TT was negatively correlated with waistline, luteinizing hormone(LH), adipose percentage of “android” area, A/G(r=-0.318、r=-0.420、r=-0.762、r=-0.319, P<0.05). Multivariate regression analysis showed that adipose percentage of “android” area may be an independent factor affecting TT. Conclusions Total testosterone level in middle-aged men with type 2 diabetes is closely related to body fat distribution. Patients with abdominal obesity are at high risk of total testosterone level decreased.
