下肢骨折患者血栓弹力图与凝血功能、炎症及血液流变学指标的相关性分析

摘要:
目的 探讨下肢骨折患者围术期血栓弹力图( TEG) 参数与凝血功能、炎症及血液流变学指标的相关性。方法 选取2018年11月至2020年7月广西壮族自治区人民医院骨科收治的81 例下肢骨折患者为研究对象进行回顾性分析,收集患者的住院期间一般资料、 TEG(R值、K值、α角、MA值)和凝血常规(PT、APTT、FIB、PLT)指标、炎症指标[C反应蛋白(CRP) 、白细胞计数(WBC) ]、血液流变学指标[血沉(ESR) 、红细胞压积( HCT) ]。采用Pearson 相关系数分析TEG 与常规凝血功能指标、血液流变学指标、炎性指标之间的相关性。结果 相关性分析显示 APTT、FIB水平分别与R值和 MA 值均呈显著正相关(r值分别为 0.287、0.319、0.310和0.550,P 均<0. 01) ,PLT与 α 角、MA 值呈显著正相关( r 值分别为 0.347和0.626, P<0. 01),与K值呈显著负相关( r =-0.318,P<0.01) ; CRP水平与MA值呈显著正相关( r = 0.351,P<0. 01);ESR与R值、 α角、MA 值呈显著正相关( r 值分别为 0.300、0.258和0.691,P<0. 01),而HCT与 α角 、MA 值呈显著负相关(r 值分别为 -0.431和-0.420,P<0. 01),与K值呈显著正相关( r = 0.471,P<0. 01)。 结论 下肢骨折患者血栓弹力图与凝血常规指标、CRP、血液流变学指标均具有一定的相关性。血栓弹力图对评估下肢骨折患者围术期凝血功能、炎症及血液流变状态均有实用意义。

Correlation of thromboelastography with coagulation function,inflammatory and hemorheology in patients with lower extremity fractures

Abstract:
Objective To investigate and compare the correlation between thromboelastography(TEG) and coagulation function, inflammatory and hemorrheology index of patients with lower extremity fractures during perioperative.Methods From November 2018 to July 2020,a retrospective study was carried out on 81 lower extremity fracture patients admitted to the Department of Orthopedics, Guangxi Zhuang Autonomous Region People's Hospital . General data of patients during hospitalization, TEG(R value, K value, α-angle, MA value) and routine coagulation function index (PT, APTT, FIB, PLT), inflammatory index [C-reactive protein (CRP) ,White blood cell count (WBC)] and hemorheology index [ESR, hematocrit (HCT)] were collected. Pearson correlation coefficient was used to analyze the correlation between TEG and conventional coagulation function index, hemorheology index, inflammatory index. Results Correlation analysis showed that APTT, FIB level were significantly positively correlated with R value and MA value respectively (r values were 0.287, 0.319, 0.310 and 0.550 respectively, P<0.01), PLT was significantly positively correlated with α Angle and MA (r values were 0.347 and 0.626 respectively, P<0. 01), significantly negatively correlated with K value (r =-0.318, P<0.01); CRP level was significantly positively correlated with MA value (r= 0.351, P<0. 01); The ESR of patients with lower extremity fractures was positively correlated with R value, α Angle, MA (r values were 0.300, 0.258 and 0.691 respectively, P<0.01); There was a significant negative correlation between HCT and MA value, α Angle (r values were -0.431and -0.420 respectively, P<0.01), which was significantly positively correlated with K value (r = 0.471, P<0.01). Conclusion The thromboelastography of patients with lower extremity fractures has a certain correlation with coagulation parameters,CRP, and hemorheology indicators,and it has practical implications for assessing coagulation,inflammation,and hemorheological status in patients with lower extremity fractures.
