血清Lp-PLA2、GGT与冠心病及冠脉病变严重程度的相关性分析

摘要:
目的 分析血清脂蛋白相关磷脂酶A2(Lp-PLA2)、-谷氨酰转移酶(GGT)与冠心病(CHD)及冠脉病变严重程度的相关性。方法 选取2018年6月至2019年6月于郑州大学第二附属医院心内科就诊的疑似CHD患者170例作为研究对象,根据冠脉造影检查结果分为冠心病组(CHD组,103例)和非冠心病组(CON组,67例)。检测不同冠脉病变严重程度的CHD患者血清中Lp-PLA2、GGT水平的变化,探讨血清Lp-PLA2、GGT与CHD及冠脉病变严重程度的关系。 结果 CHD组患者血清中Lp-PLA2、GGT、甘油三酯(TG)、低密度脂蛋白(LDL)、Gensin积分、高血压和糖尿病比例明显高于CON组,而高密度脂蛋白(HDL)明显低于CON组(均P0.05)。随着冠脉病变支数的增加及冠脉病变严重程度的加重,血清中Lp-PLA2、GGT水平均逐渐增加,两两比较差异均具有统计学意义(均P0.05)。相关性分析显示CHD组患者血清中Lp-PLA2、GGT与冠脉病变支数(rs= 0.681、603,均P 0.001)及Gensin积分(r= 0.799、0.621,均P 0.001)呈明显的正相关。多因素分析显示Lp-PLA2(OR= 1.802)、GGT(OR= 1.707)、高血压(OR= 2.697)、LDL(OR= 1.595)均是CHD的危险因素,HDL(OR= 0.423)是CHD的保护因素(均P0.05)。 结论血清Lp-PLA2、GGT是CHD的发病的高危因素,且二者可较好的反映冠脉病变严重程度,可作为评估CHD发生、进展的可靠指标。

Correlation analysis of serum LP-PLA2 and GGT with the coronary heart disease and severity of coronary artery disease

Abstract:
ObjectiveTo analyze the correlation between serum lipoprotein-associated phospholipase A2 (Lp-PLA2), Gamma-glutamyl Tansferase (GGT) with the coronary heart disease (CHD) and severity of coronary artery disease.Methods A total of 170 suspected CHD patients admitted to the cardiology department of the Second Affiliated Hospital of Zhengzhou University from June 2018 to June 2019 were selected as study subjects. According to the results of coronary angiography, they were divided into a coronary heart disease group (CHD group, 103 cases) and a non-coronary heart disease group (CON group, 67 cases).The changes of LP-PLA2 and GGT levels in serum of CHD patients with different severity of coronary artery disease were detected, and the relationship between serum LP-PLA2 and GGT with the severity of CHD and coronary artery disease was discussed.Results The serum levels of LP-PLA2, GGT, triglyceride (TG), low density lipoprotein (LDL), Gensin score, hypertension and diabetes in CHD group were significantly higher than those in CON group, while high density lipoprotein (HDL) was significantly lower than that in CON group (all P0.05).With the increase of the number of coronary lesions and the aggravation of the severity of coronary lesions, the levels of LP-PLA2 and GGT in serum increased gradually, and pairwise comparison showed statistically significant differences(all P0.05).Correlation analysis showed that LP-PLA2 and GGT were positively correlated with the number of coronary lesions (rs=0.681, 603, all P0.001) and Gensin score (r=0.799, 0.621, all P0.001) in the CHD group.Multivariate analysis showed that LP-PLA2 (OR=1.802), GGT (OR=1.707), hypertension (OR=2.697), LDL (OR=1.595) were all risk factors for CHD, and HDL (OR=0.423) was the protective factor(all P0.05).Conclusion Serum LP-PLA2 and GGT are high risk factors for CHD, and they can better reflect the severity of coronary lesions, and can be used as reliable indicators to evaluate the occurrence and progression of CHD
