静态进展性牵伸支具治疗创伤术后肘关节僵硬患者的临床疗效观察

摘要:
目的:探讨静态进展性牵伸支具对创伤术后肘关节僵硬患者的关节活动度及功能的临床效果。方法:回顾56例创伤后肘关节僵硬患者,支具组28例,对照组27例。支具组和对照组均进行常规的康复治疗(包括蜡疗,超声,关节松动术、软组织松解、冷敷),支具组常规康复配合静态进展性牵伸支具的治疗。评估治疗前、后和治疗后随访半年的屈、伸肘角度变化,Mayo评分的改变,记录治疗周期时长。结果:治疗后及半年随访时,两组患者屈/伸肘角度均较治疗前有显著改善,p<0.05;Mayo评分显著提高,p<0.05。支具组屈伸角度改善优于对照组,p<0.05,治疗周期短于对照组。结论:应用静态进展性牵伸支具对于创伤术后肘关节僵硬患者的关节活动度改善及功能改善均具有显著促进作用。

Effect of static-progressive stretch splinting in the treatment of posttraumatic elbow stiffness

Abstract:
Objective: To investigate the clinical effect of static-progressive stretch splinting on the range of motion and function of elbow joint stiffness after trauma surgery. Methods: 56 cases of elbow stiffness after trauma were reviewed, including 28 cases in splint group and 27 cases in control group. Both the splint group and the control group received convention rehabilitation treatment (including wax therapy, ultrasound, joint mobilization, soft tissue release and cold therapy), while the splint group was treated with static-progressive stretch splinting. The changes of elbow flexion and extension angle, Mayo score and treatment period were evaluated. Results: Compared to the pre-treatment , the elbow flexion/extension angle of the two groups were significantly improved after treatment and six months after treatment, p<0.05; Mayo score was significantly increased after treatment and six months after treatment, p<0.05. The improvement of flexion and extension angle in the splinting group was better than that in the control group, p<0.05, and the treatment period was shorter than that in the control group. Conclusion: the application of static-progressive stretch splinting can significantly improve the range of motion and function of elbow joint stiffness patients after trauma surgery.
