| 目的：研究头皮鳞癌侵袭至骨膜时，减少肿瘤复发的方法。方法：南京中医药大学附属鼓楼临床医学院2010年至2020年共收治13例头部鳞癌侵袭骨膜的患者。通过整理入组患者的病例、影像、病理等资料，明确手术治疗方式为皮肤肿瘤切除术+颅骨外板磨削术+局部皮瓣转移术+植皮术。结果：13例患者术后均恢复良好，随访一年，10例患者生存状态良好，无复发及转移迹象，疗效满意。1例患者多处复发转移，半年后去世；2例患者植皮区成活欠佳，二期手术后愈合良好。结论：当头皮鳞癌侵袭至骨膜时，颅骨外板部分或全部磨除可有效减少肿瘤的复发。 |

| From 2010 to December 2020, Gulou Clinical Medical College affiliated to Nanjing University of Traditional Chinese Medicine , treated thirteen patients with head squamous cell carcinoma invading the periosteum. Objective: To study the methods to reduce tumor recurrence when scalp squamous cell carcinoma invades the periosteum. Methods: By sorting out the case, imaging, and pathological data of the enrolled patients, we use the surgical treatments including skin tumor resection,skull outer plate grinding,local flap transfer and skin grafting. Results: Both patients recovered well after the operation,and were followed up for one year.10 patients were in good living condition, no signs of recurrence and metastasis, and the curative effect was satisfactory. One patient had multiple recurrences and metastases, and died half a year later; two patients had poor survival in the graft area, and healed well after the second-stage operation. Conclusion: When scalp squamous cell carcinoma invades the periosteum, partial or complete removal of the outer skull plate can effectively reduce tumor recurrence. |
