VI-RADS评分在膀胱癌肌层浸润中的价值

摘要:
目的 研究膀胱影像报告和数据系统(VI-RADS)联合多参数磁共振(mp-MRI)在膀胱癌肌层浸润中的价值。方法 回顾性分析安徽医科大学附属滁州医院2018年5月到2022年7月的膀胱癌患者59例,根据术后病理,将患者分为肌层浸润性膀胱癌(MIBC)和非肌层浸润性膀胱癌(NMIBC)组,由两位影像科医生,独立对这些病人图像按照VI-RADS的评分标准进行评分,意见不一致时,沟通达成一致,再将VI-RADS的评分与病理结果作对比,从而评估VI-RADS评分联合mp-MRI对MIBC的诊断价值。应用受试者工作特征(ROC)曲线分析VI-RADS评分对膀胱癌肌层浸润的诊断效能。结果 两位评分者的 VI-RADS评分一致性良好(符合率为81.67%,kappa值=0.755,P<0.001)。ROC曲线分析显示:当VI-RADS评分≥3.5时,诊断MIBC的曲线下面积为0.880(95%CI 0.790-0.971),约登指数最大为0.584,敏感度63.64%,特异度94.74%。结论VI-RADS评分对于膀胱癌肌层浸润的准确性较高,值得在临床推广。

Value of VI-RADS score in myometrial invasion of bladder cancer

Abstract:
Objective To study the value of bladder imaging reporting and data system (VI-RADS) combined with multi-parameter magnetic resonance (mp-MRI) in the myometrial invasion of bladder cancer. Methods Methods 59 patients of Chuzhou Hospital of Anhui Medical University from May 2018 to July 2022, patients were divided into myometrial invasive bladder cancer (MIBC) and non-myometrial invasive bladder cancer (NMIBC). Two imaging doctors independently scored the scoring criteria of VI-RADS, and compared the score of VI-RADS and pathological results to evaluate the diagnostic value of VI-RADS score and mp-MRI on MIBC. The effectiveness of the infiltration in the diagnosis of VI-RADS score was analyzed using the subject working characteristic (ROC) curve. Results The VI-RADS scores were well (compliance rate was 81.67%, kappa value =0.755, P <0.001). ROC curve analysis showed that when the VI-RADS score was 3.5, the area under the curve of MIBC was 0.880 (95%CI 0.790-0.971), the maximum yoden index was 0.584, the sensitivity was 63.64%, and the specificity was 94.74%. Conclusion The accuracy of VI-RADS score for bladder cancer is worth clinical promotion.
