| 目的 探讨耳内镜下采用不同材料进行鼓膜修补术的效果。方法 以71例 (71耳)鼓膜穿孔患者为研究对象。按修补材料不同分为3组,其中一组采用耳屏软骨膜(10例)、 一组采用耳屏软骨(41例),另一组采用颞肌筋膜(20例),统计各组的手术指标、术后听力恢复情况、随访并发症、鼓膜愈合及再穿孔发生情况。结果 术后随访6个月,耳屏软骨组的术后再穿孔率较其于两组小,差异均具有统计学意义(p<0.05),手术时间、术后平均气骨导差缩小值及平均气导听阈缩小值3组均无明显差异(P>0.05),颞肌筋膜组住院时长较其他两组明显延长,差异均具有统计学意义(p<0.05)。结论 耳内镜下鼓膜修补术中采用耳屏软骨修补,术后效果好,可降低术后再穿孔率,手术操作较易,易于推广。 |

| Objective To compare the clinical effect of tympanic membrane perforation repair with different materials．Methods A total of 71 patients(71 ears) with tympanic membrane perforation repaired under endoscope ．All The patients were divided into three groups according to the repair material. 10 cases used temporal muscle fascia as repair material, 41 cases used tragus cartilage and 20 cases used tragus perichondrium．The operation station,the effect of hearing improvement after operation as well as the success rate of tympanic membrane repair were compared between the three groups．Results All the patients had been followed up for at least 6 months．The success rate of tympanic membrane repair of the group used tragus cartilage as material were less than the other two groups(p<0.05).There were no significant differences between all the groups in the operation time, postoperative average air-bone gap(ABG)and average air conduction hearing threshold (PTA)(p<0.05). The length of hospital stay of the group used tragus perichondrium as material were longer than the other two groups(p<0.05)．Conclusion Among the materials for tympanoplasty repair under otoendoscopy ,the tragus perichondrium cartilage has shorter operation time and good postoperative effect, which will reduce the rate of postoperative reperforation, and easy to operation and popularize. |
