依达拉奉在心脏不停跳手术中对C3、IL-6及IL-8的影响

摘要:
[摘要] 目的 观察依达拉奉在心脏不停跳手术基础上对C3、IL-6、IL-8的影响。方法 将60例需行二尖瓣置换术的风湿性心脏病患者随机分成两组:实验组(E组)30例,对照组(C组)30例。E组于预充液中按0.5 mg/kg加入依达拉奉,C组加入等量生理盐水。分别于术前、转机30 min、停机、术毕、术后24 h、术后72 h各时点观察血清C3、IL-6、IL-8含量的变化。结果 (1)C3浓度水平随体外循环(CPB)及手术时间逐渐下降,均于手术结束时达最低谷,术后24 h开始升高,术后72 h恢复至术前水平,两组之间浓度水平差异无统计学意义(P>0.05)。(2)IL-6、IL-8浓度水平随CPB及手术时间逐渐增高,均于手术结束时达高峰,术后24 h下降,术后72 h恢复至术前水平,两组之间浓度水平差异有统计学极显著意义(P<0.01),实验组低于对照组。结论 依达拉奉在心脏手术中可减少炎性因子的生成,减轻术后全身炎性反应。

Effects of edaravone on plamas levels of C3,IL-6 and IL-8 in patients undergoing on-pump beating heart surgery

Abstract:
[Abstract] Objective To investigate the effect of edaravone on plamas levels of C3,IL-6 and IL-8 in patients undergoing on-pump beating heart surgery. Methods Sixty patients with rheumatic heart disease undergoing mitral valve replacement were divided into two groups: edaravone group (group E, n=30): patients received edaravone injection 0.5 mg/kg in circuit priming fluids; Control group(group C, n=20): patients received equal volume of normal saline. C3, IL-6 and IL-8 were respectively measured at six different time intervals (preoperation, 30 minutes after CPB, termination of CPB, postoperation, 24 hours and 72 hours after postoperation). Results (1)The levels of plasma C3 in both groups were declined from the beginning of CPB, reached trough at postoperation, elevated from the time of 24 hours after postoperation, restored to normal at the time of 72 hours after postoperation, there is no statistical difference between groups (P>0.05). (2)The levels of plasma cytokines in both groups were elevated from the beginning of CPB, reached peak at postoperation, declined from the time of 24 hours after postoperation, restored to normal at the time of 72 hours after postoperation, the levels of edaravone group were lower than those of control group(P<0.01). Conclusion Edaravone can decrease the plasma levels of inflammatory cytokines such as IL-6 and IL-8, alleviate the postoperative systemic inflammatory response in cardiac surgery.
