| ［摘要］ 目的 观察泪道激光联合泪道环形置管治疗难治性上泪道阻塞的疗效。方法 采用随机对照研究方法，按就诊顺序排序号，随机将74例难治性上泪道阻塞患者分为两组，A组34例采用单纯激光治疗。B组40例采用激光联合环形置管治疗，患者行KTP-Nd:YAG激光泪道成形术，然后硅胶环形泪道置管,保留3个月后拔管。观察两组术中、术后及拔管后随访期间溢泪症状变化及泪道冲洗情况，同时记录并发症。结果 治疗结束后3个月的随访情况表示，A组治愈率为35.3%,好转率为17.6%,总有效率为52.9%；B组治愈率为65.0%,好转率为25.0%,总有效率为90.0%（P<0.01）。两组均无术中并发症，B组术后3个月拔管时发现3例泪小点息肉。结论 泪道激光联合泪道环形置管治疗难治性上泪道阻塞创伤小、疗效好、操作简单，易于推广。 |

| ［Abstract］ Objective To evaluate the effect of the severe upper lacrimal duct obstruction treated by laser combined with annular silicone intubation.Methods Seventy-four patients with severe lacrimal duct obstruction were divided randomly into two groups.Group A(34 eyes) were treated by laser.Group B(40 eyes) were treated by laser combined with annular silicone intubation,the silicone tube was kept in place for 3 months after surgery.The epiphora symptom changes,lacrimal conditions and the complications were observed during intraoperative,postoperative and the follow-up period after extubation.Results The effects were compared between two groups affter 3 months operation.In group A:12 eyes(35.3%) were cured,6 eyes were improved,the total effective rate was 52.9%.In group B: 26 eyes(65.0%) were cured,10 eyes were improved,the effective rate was 90.0%(P<0.01).There were no intraoperative complications of two groups.Three cases of lacrimal point polyps were found in B group at 3 months after extubation.Conclusion Laser combined with annular silicone tube intubation is an effective,simple and easy way to promote in the treatment of upper lacrimal duct obstruction. |
