| ［摘要］ 目的 探讨基层医院在有限条件下进行扁桃体切除的有效手术方法。方法 总结分析在全麻下应用改良戴氏开口器+低能量电凝模式下切除153例扁桃体的资料及该术式的优缺点。结果 153例手术时间8～15 min，术中出血量5～10 ml，平均7.5 ml。术后1 d观察两侧扁桃体窝伪膜生长良好，24 h内无出血，3例在术后1周因伪膜脱落发生出血，经局部压迫等对症治疗后好转。术后住院1～4 d,平均2.5 d。术后观察2周151例患者创口愈合良好,2例术后1周出现创口感染，给予抗炎及局部雾化吸入治疗后好转。结论 改良戴氏开口器+低能量电凝模式下扁桃体切除术可明显减少手术出血量、缩短手术时间，值得在基层医院推广应用。 |

| ［Abstract］ Objective To explore how to better perform tonsillectomy in basic hospital under the existing condition.Methods The experience on application of modified Dai′s mouth-gag combined with low energy electric coagulation pattern in 153 patients with tonsillectomy under general anesthesia were summarized.Results In 153 patients, the surgery time were about 8～15 minutes, the intraoperative bleeding were about 5～10 ml, with an average of 7.5 ml.The growth of false membrane in the both sides amygdaline depression was good one day after operation.No bleeding was found during 24 h after operation, bleeding due to false membrane shedding were found in 3 patients one week after operation, which were improved after the partial pressure and other symptomatic treatment. The postoperative hospital stay were about 1～4 d, the average 2.5 d. The good wound healing were obtained in most patients after two week operation except wound infections were found in 2 patients one week after operation,which were improved after anti-inflammation and the partial atomization inhalation treatment.Conclusion The application of modified Dai′s mouth-gag combined with low energy electric coagulation pattern in the tonsillectomy can reduce the intra operative bleeding obviously, shorten the surgery time obviously, and is worth popularizing in the basic hospital. |
