| ［摘要］ 目的 分析经腹腔镜手术治疗的卵巢成熟型畸胎瘤病例的临床特点。方法 对106例经腹腔镜手术治疗的卵巢成熟型畸胎瘤患者的临床资料进行回顾性分析。结果 106例患者均在腹腔镜下完成手术，无严重并发症发生。彩色多普勒超声诊断符合率为98.11%（104/106）。手术中畸胎瘤破裂组比未破裂组手术时间长、出血量多，两组比较差异有统计学意义（P<0.05或P<0.01）；而术后复发率、术后妊娠率两组比较差异无统计学意义（P>0.05），术中行对侧卵巢剖探25例，未见异常。对侧卵巢剖探对术后妊娠率的影响与未行剖探组（81例）比较差异无统计学意义（P>0.05）。结论 腹腔镜手术治疗卵巢成熟型畸胎瘤技术成熟，安全可靠。术中畸胎瘤破裂对术后复发率、术后妊娠率未见明显影响。对侧卵巢剖探并不需要常规进行。 |

| ［Abstract］ Objective To analyze the clinical characteristics of patients with mature ovarian teratoma who received laparoscopic treatment.Methods The clinical data of 106 patients with mature ovarian teratoma who received laparoscopic surgery were analyzed retrospectively.Results All of 106 patients received laparoscopic surgery successfully and had no serious complications. The accuracy of diagnosis by color doppler ultrasonography was 98.11%(104/106). The rupture of mature ovarian teratoma during laparoscopic surgery had no significant impact on the postoperative pregnant rate and the postoperative morbidity (P>0.05), while had significant impact on the volume of blood loss and the time of operation (P<0.05). Examination of the other ovary was performed during the operation in 25 patients, but no lesion was found. The examination of the other ovary during the operation had no significant impact on the postoperative pregnant rate of patients (P>0.05).Conclusion Laparoscopic treatment of mature ovarian teratoma is a mature,safe and reliable technology. The rupture of mature ovarian teratoma during laparoscopic surgery had no significant impact on the postoperative pregnant rate and the postoperative morbidity. Examination of the other ovary during the operation had no significant impact on the postoperative pregnant rate. If the appearance of the other lateral ovary is normal, exploratory operation is unnecessary. |
