腰椎椎体后缘软骨结节致侧隐窝狭窄的CT诊断

摘要:
[摘要] 目的 探讨腰椎椎体后缘软骨结节(LPMN)致侧隐窝狭窄的CT诊断价值。方法 2008~2010年对具有腰腿痛症状的3 000例无明显外伤患者进行CT腰椎检查,筛选出具有LPMN的84例病例进行分析。结果 LPMN有特征性的CT表现为腰椎椎体后缘类圆形或不规则形骨质缺损区,大小不一,密度与椎间盘基本相同,边缘有厚薄不一的硬化,骨质缺损后方可见条形或弧形骨块并突入椎管内,全部或部分与椎体分离,椎管狭窄,硬膜囊受压,其中95.2%(80/84)合并有同层的椎间盘突出,造成侧隐窝不同程度的影响。软骨结节具有单发及多发,其中大多数影响到侧隐窝变窄,侧隐窝狭窄程度分为轻、中、重度。结论 CT扫描能发现腰椎椎体后缘软骨结节,及其对侧隐窝影响的情况,并能评估侧隐窝狭窄程度,对提高侧隐窝狭窄的诊断具有一定的临床意义。

Abstract:
[Abstract] Objective To evaluate the value of CT in the diagnosis of lateral recess stenosis due to lumbar posterior marginal intraosseous cartilaginous node (LPMN).Methods CT scanning of lumbar vertebrae was performed in 3 000 patients aching in the loins and legs from 2008 to 2010.Of them 84 cases with LPMN were selected and their clinical and CT manifestation were studied.Results The characteristic CT manifestation of LPMN showed: Round and irregular bony defect of uneven size, its density being similar to that of lumbar intervertebral disc, un-even sclerosis appearing on the edge, bar or ach shapes of bones bursting into the vertebral canal, all or part departing from the centrum, narrow vertebral canal, hard membrane vesicle being pressed; Most cases had combined lumbar intervertebral disc protrusion caused influences on lateral recess in various degree; single or multiple cartilaginous nodes was found and most of which caused lateral recess stenosis slightly, moderately or severely and remarkable differences existed.Conclusion CT scanning can find LPMN and the its influences on lateral recess. It can also evaluate the stenosis degrees of lateral recess and have clinical significance in raising the diagnosis level of the lateral recess stenosis.
