壳聚糖宫颈抗菌膜治疗宫颈病变的临床研究

摘要:
[摘要] 目的 探讨壳聚糖宫颈抗菌膜在宫颈病变治疗后促进创面愈合的效果。方法 将2009-05~2010-05经宫颈细胞学检查无异常的单纯宫颈炎症患者300例和经宫颈细胞学及HPV-DNA-阴道镜-宫颈活检三阶梯方法证实的宫颈上皮内瘤变CINⅠ~Ⅱ级患者200例为研究对象。按数字随机法随机分为实验组(常规治疗+壳聚糖宫颈抗菌膜治疗)和对照组(单纯常规治疗),观察术后阴道流血量及持续时间、阴道流液时间、创面愈合时间并进行统计分析。单纯宫颈炎患者用激光治疗,宫颈上皮内瘤变CINⅠ~Ⅱ级采用宫颈环形电切术(LEEP)治疗。结果 实验组激光术后阴道流血量较对照组明显减少(P<0.01);阴道流血持续时间、阴道排液时间、宫颈创面愈合时间较对照组明显缩短(P<0.01);LEEP术后阴道流血量较对照组明显减少(P<0.01);阴道流血持续时间、阴道排液时间、宫颈创面愈合时间较对照组明显缩短(P<0.01)。结论 物理治疗后联合应用壳聚糖抗菌膜,可明显减少术后阴道流血量,缩短术后阴道排液及阴道流血时间,缩短创面的愈合时间,减轻了患者的痛苦,提高患者的生活质量。

Clinical studies of Chitosan cervical antimicrobial films in the treatment of cervical lesions

Abstract:
[Abstract] Objective To improve the healing process after treatment on cervical lesion.Methods Three hundred patients with single cervicitis and normal cytological examination results were collected between May 2009 and May 2010. Another 200 patients were confirmed to be cervical intraepithelial neoplasia (CIN)Ⅰ~Ⅱ by cervical cytology, HPV-DNA- colposcopy- Cervical biopsy. They were randomized divided into experimental group(routine treatment+Chitosan cervical antimicrobial films treatment) and control group with routine treatment only. The amount and duration of vaginal bleeding, duration of fluids from vagina and duration of healing of the wound were observed and analyzed. The patients with single cervicitis were treated with laser and those with CINⅠ~Ⅱ were treated with loop electrosurgical excision procedure (LEEP).Results Compared with control group,the vaginal bleeding obvious reduced(P<0.01). The durations of vaginal bleeding, fluids from virgin and healing of the wound were significantly shortened after laser in the experimental group. The was statistically difference (P<0.01). The virginal bleeding in the experimental group was significantly less than those of the control group after LEEP(P<0.01) The durations of vaginal bleeding, fluids from vagina and healing of the wound in the experimental group were significantly shorter than those in the control group after LEEP. The was statistically difference(P<0.01).Conclusion Physiotherapy combined with Chitosan cervical antimicrobial films can significantly reduce virginal bleeding and shorten the vaginal bleeding, fluids from virgin and healing of the wound without complications, which relieves pains and improve life quality of the patients.
